Staff member #1 made child #1 cry due to the staff members harsh tone of voice. Requirement: A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: Staff person #1 has been switched to a new role that will alleviate stress, was coached on being aware of tone of voice, and was encourages to seek emotional support. Promote development (3270.111(c))

Staff member #1 stated that they placed their hand on child #1 chin to redirect the child to listen. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: Staff person #1 was coached on appropriate vs inappropriate physical contact with children. Administration will also be observing this staff member's interactions with children regularly. Emergency plan (3270.27(a)(6))

An emergency drill was not conducted at least once annually. Emergency drill was documented to have lapsed 8/30/2024 and 11/19/2025. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: The next emergency drill and intruder training is scheduled prior to November 2026. Inspection: Renewal. Building Surfaces (3270.76)

It was observed in the young toddler classroom, a build-up of dirt, food and debris on the stepstool used by young toddlers for handwashing. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The stepstool was thoroughly cleaned on all sides. Cleaning checklists are in every classroom. The stepstool specifically, has been added to the cleaning checklist in this classroom to ensure it is not overlooked. Inspection: Renewal. Original container (3270.66(b))

It was observed in the infant classroom a Magic Eraser placed in a cabinet under the sink was not in its original container or in a container that specifies the content. (This was corrected at the time of inspection) Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: Corrected onsite. Age and Training (3270.31(e)(4)(i))

Staff person #2 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring 10/11/2023. Documentation of updated Pediatric first aid and CPR training is not on file. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required training has been completed, staff person #2 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #2, staff person #2 may not work in a child-care position at the facility. Correction: Staff person #2 will be completeing the required CPR training on 3/10/26 through a PQUAS certified trainer. Inspection: Renewal. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Operability of the fire detection devices were not tested every 30 days. Testing was documented to have lapsed 4/1/25 - 5/2/25. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Small Toys and Objects (3270.103)

It was observed on the young toddler outside playground, a significant number of small rocks less than 1 inch in diameter were inside the fenced area on the turf, inside a playhouse and inside a shopping cart push toy accessible to children who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Clean and good repair (3270.102(a))

It was observed on the outdoor playground that the black coating on the steps of the imbedded climber was broken exposing rust and creating pinch points and rough edges causing potential injury to children. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The slides steps have been sealed with duct tape to remove pinch points and sharp edges. New steps/slide platform have been ordered and will be installed in July. The current Chile Abuse Clearance is dated 1/30/25 and the previous was dated 11/22/19. The file for staff person #2 included a State Police Clearance and Child Abuse clearance which was not updated within 60 months of the previous clearance. The current State Police Clearance is dated 7/8/24 and the previous was dated 6/6/19. The current Child Abuse clearance is dated 7/16/24 and the previous was dated 7/5/19. The file for staff person #3 included a NSOR Certificate which was not updated within 60 months of the previous clearance. The current NSOR Certificate is dated 1/24/25 and the previous was dated 11/21/19. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).
