Requirement: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. An agreement shall specify the date of the child's admission. Correction: Fee agreement and emergency contact forms were on file for child # 2 on 2/13/26. Child # 1 & # 2 do not have signed consent for emergency medical care and minor first aid. Requirement: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Correction: Child # 1 & # 2 will have consents signed for emergency medical care and minor first aid when they arrive to pick up children on 3/24/26. Locked or inaccessible - in original container (3290.64(a))

There is a storage container of food and snacks on the floor of the bathroom used by the children on the first floor along with floor cleaning products. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Requirements (3290.74(a))

The children's bathroom on the first floor has a cabinet with a metal wire hanging from the cabinet to where the children can reach and is accessible to them. The kitchen has clutter on the microwave stand and a small table near the basement door with visible hazards accessible to the children. There are visible hazards such as glue sticks and scissors that are in the toy bins in the childcare room accessible to all children including those still placing objects in their mouths. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Correction: The wire was removed, the microwave stand and clutter was removed. All hazards were removed from the childcare space. Inspection: Initial review. Small Toys and Objects (3290.103)

There are hot wheel cars with tires less than 1 inch in diameter in the toy bins used by all the children in the childcare space. There is a thumb tack on the couch in the childcare room to be used by all the children. There are plastic bags in a bin on the childcare changing table accessible to the children. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The toys were all placed in bags that will be given to age-appropriate children that are not in the room with small children placing objects in their mouths. Inspection: Initial review. Locked (3290.133(4))

There are multiple bottles of prescription medication for the family on top of the refrigerator in the kitchen accessible to the children that will be eating in the kitchen for meals and walking through to use the bathroom. There is prescription medication unlocked in the refrigerator accessible to the children. Requirement: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children. Correction: A lock was placed on the refrigerator. System in compliance/Operator or designated staff responsibility (3290.95(a)/3290.95(b))

The 10-year lithium-ion wireless fire alarms are not interconnected with the second-floor alarms when sounded off. Only the first and second floor interconnect and sound off when tested with each other. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: Operator purchased and installed approved and appropriate fire alarms and smoke detectors. The alarms are wireless and interconnected and are heard on each floor.
