Facility fire alarms have not been tested in over 30 days. Last tested on 1/22/26. Facility staff # 1 reported to certification rep during opening inspection that they have only resided in PA for 8 months now. Facility staff # 1 does not have any clearances on file that are required from New York the state facility staff # 1 moved from 8 months ago. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
