In the Preschool Room, two containers of disinfecting wipes were stored on their side, above a microwave which is used to prepare food for children. To the left of the rear exit in the Preschool Room, an unlocked utility closet was observed to contain an open bag of sidewalk salt, which is labeled "keep out of reach of children." In the Toddler Two Room, cleaning supplies were stored above the microwave, which is used to prepare food for children. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: In the preschool room, the 2 containers of disinfecting wipes were moved to a different location away from the cabinet above the microwave. A child lock was placed onto the closet door with the sidewalk salt. The Toddler room cleaning supplies were [put] into a plastic bin. Inspection: Renewal. No peeling paint or plaster (3270.77(a))

In the Preschool Room, multiple areas of damaged plaster were observed on the green wall to the right of the front window. These damaged areas continued around the corner under the reading area and "All About Me" postings. In the same room, plaster damage was observed under the bulletin board, and a large crack was observed in the kitchenette, under the paper towel dispenser. In the bathroom at the rear right of the Preschool Room, damaged plaster was observed to the right of the toilet. In the Toddler Two room, damaged plaster was observed to the right of the microwave and garbage can. Also in the Toddler Two room, an area of damaged plaster was observed on the wall near the planets display. In the Gym, a small area of damaged plaster was observed at the bottom of the wall, under the words "School Age." In the School Age room, a long crack was observed in the plaster on the orange wall near the reading chair, and the plaster appeared to be separating. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: A contractor was hired to fix all of the damaged walls throughout the center. Duct tape was put over the rough spot in the gym. Inspection: Renewal. Clean and good repair (3270.102(a))

In the School Age room, a toy bin along the front window area was observed to have a broken hinge. This caused the lid to be unable to hinge open, instead slamming shut easily. This creates a risk of injury to children. In the School Age room, the pool table was observed to be in poor repair. The pressed wood is peeling and separating in places, creating a risk of pinching. In the School Age room, the foosball table was observed to have chips in the wood near the red side's goal. These chips caused the surface to have a sharp, rough edge. Also in the School Age room, a clear plastic bin of building blocks was observed to have a large crack near the handle. The crack created a sharp edge, which poses risk of injury to children. In the Waddlers Room, a large wooden room divider was observed to be wobbly and unstable. Additionally, several areas of splintered wood were observed on the divider, creating sharp edges. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The toy bin in the front of the SA room had the lid removed. The foosball, pool table and block bin were taped and repaired. The wooden divider was disposed of. Inspection: Renewal. In the Toddler Two room, a wooden white shelf located near the book center was observed to have a sharp metal hinge exposed on the side. This was within reach of children and could cause injury. In the School Age room, a black and blue gaming chair in the reading area was observed to have a hole near the seat, exposing a metal piece of the frame. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: The gaming chair was taped and repaired. The Emergency Contact form for Child #1 did not include the phone number of the physician or medical care provider. Staff #2, who was responsible for the 5 children, could not be seen. When the CR asked who was supervising the children, another staff went to the back corner of the room and retrieved Staff #2. Staff #2 had been in the back of the room prepping for naptime. Staff #2 was unable to supervise the 5 children from her position in the back corner of the room. Later, Staff #2 left the 5 children, and went to the far side of the room and leaned out the door to speak to another staff located in the outdoor play space. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). TIERED LIS: 1. This portion of the plan shall have an immediate correction date. 2. The legal entity will require all staff persons at the facility including the director to complete a training regarding Supervision and Ratio. The training must be approved by the Department prior to implementation. Upon completion of the training, staff training documentation will be provided to the OCDEL-DHS representative by email. The training may count towards the mandatory 12 training hours required by the Department. The operator shall provide a date for when this training will be complete. Correction: 1. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The laundry room door was fixed in which it made the paint inaccessible to children. The sanitizer and wipes were removed from the drawer and put out of reach of children or disposed of.
