In the kitchen, there were 6 children observed with staff person #1, and there was a stove observed with knobs that were accessible to the children. Building Surfaces (3280.76)

On the right front and left side of the building, the siding was cracked in multiple areas with sharp edges that were accessible to children. In the kitchen, the door leading to the back hallway of the facility had a small hole in the door. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: I plan to immediately correct the non compliance by replacing the vinyl that's damaged on the building and covering the hole in the door. Inspection: Allocated Unannounced Monitoring. On 6-6-25 at approximately 12:20 p.m., there were three children observed sitting at a table that was behind a large pillar in the room. The staff person stated that they were sitting in a chair on the other side of the pillar. From the chair, the staff person was unable to directly see two of the three children due to the pillar obstructing their view. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: I plan to immediately correct the non compliance by bringing all children from sitting at the table behind the pillar area to make sure the children can be seen and supervised at all times.
