In the Toddler 2 room, an Epinephrine auto-injection pen for Child #1 was observed to be expired as of 12/31/2025. (SEE LIS CODE SHEET) Requirement: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label. Correction: The expired Epi Pen was removed from the medication box and was sent home with the parent that evening at pick up. Inspection: Renewal. Clean and good repair (3270.102(a))

In the Toddler Outdoor Playspace, two pieces of outdoor equipment were found to be in poor repair: 1) a multicolored ride-on toy was observed to have a broken steering wheel. The broken portion created a sharp edge, which is a potential hazard to children. 2) the cube-shaped climbing equipment with the pink slide was observed to have a crack measuring approximately 8 inches at the top of the slide. The crack created both a sharp edge and a pinch point. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The riding car toy was removed from the playground and was thrown away in the dumpster. Building Surfaces (3270.76)

In the Toddler 2 room along with light purple wall, the trim was observed to be separating from the wall behind the circle time area. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Arts and crafts non-toxic (3270.66(e))

In the Pre-K room in a gray cabinet near the emergency exit, a basket of art supplies was found to contain four ink daubers (1 Dab-o-ink, 1 Dabbin Fever and 2 Primo brand), which were marked "keep out of reach of children." Requirement: Arts and crafts materials shall be nontoxic. Correction: All of the daubers that were marked "keep out of reach of children" were thrown away. Inspection: Renewal. The file for Staff Person #1 contained an incomplete State Police Clearance. The clearance was requested on 6/3/2025, but the printed copy lacked the dissemination date. Correction: I obtained the correct/completed State Police clearance that included the dissemination date for that staff member. Inspection: Renewal. No ridicule or threaten harm/No harsh language (3270.113(c)/3270.113(d))

During the course of a complaint investigation, Staff Person #1 admitted to taking Child #1 into a facility bathroom while the child was having behavioral issues. While in the bathroom, Staff Person #1 admitted to yelling loudly at the child, threatening them with physical harm. Two other enrolled children were present in the classroom area when this occurred. Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: The day of the incident, staff person #1 was pulled aside by the director and assistant director. We told [them] what heard and asked for [their] input on what happened. After [they] explained, [they were] told that as a staff member, that behavior isn't allowed, even if the child is [their] own. [They were] told that other children were witnesses as well which is also unacceptable. [They] agreed and apologized. Inspection: Unannounced Monitoring. The State Police clearance on file for staff person #3 was missing a dissemination date. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. The health assessment on file for staff person #1 is missing a check in the box stating that they are suitable to provide childcare. Requirement: An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care. Correction: Staff #1 had her doctor fix the unchecked box stating that she is suitable to provide childcare. The doctor initialed and dated the mistake 3/7/25. Inspection: Renewal. Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)(10))

Staff #1 has a pediatric first aid/CPR training on file that is not an approved curriculum within 90 days of hire. Requirement: Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Correction: Staff person #1 attended a pediatric 1st aide/CPR training on 3/5/25 with an approved PQAS instructor and with the correct adult and pediatric training. Inspection: Renewal. Professional development certificate (3270.31(g))

Staff person #1 and Staff person #2 had CPR/First Aid on file that was not a PQAS approved Pediatric First Aid/CPR course taught by a PQAS approved instructor within 90 days of their hire date. Requirement: Completion of professional development shall be documented by the signature and title of a representative of the professional development entity and include the date professional development was completed. Documentation shall be retained in the facility person's file or maintained in an electronic system as designated by the Department. Documentation of the completion of the professional development under subsection (f) taken from September 30, 2016, forward satisfies this requirement. Correction: Staff #1 and staff #2 attended a pediatric 1st aid/CPR training on 3/5/25 with an approved PQAS instructor and with the correct adult and pediatric training. Inspection: Renewal.
