In the Toddler Outdoor Playspace, two pieces of outdoor equipment were found to be in poor repair: 1) a multicolored ride-on toy was observed to have a broken steering wheel. The broken portion created a sharp edge, which is a potential hazard to children. 2) the cube-shaped climbing equipment with the pink slide was observed to have a crack measuring approximately 8 inches at the top of the slide. The crack created both a sharp edge and a pinch point. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The riding car toy was removed from the playground and was thrown away in the dumpster. During the course of a complaint investigation, Staff Person #1 admitted to taking Child #1 into a facility bathroom while the child was having behavioral issues. While in the bathroom, Staff Person #1 admitted to yelling loudly at the child, threatening them with physical harm. Two other enrolled children were present in the classroom area when this occurred. Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: The day of the incident, staff person #1 was pulled aside by the director and assistant director. We told [them] what heard and asked for [their] input on what happened. After [they] explained, [they were] told that as a staff member, that behavior isn't allowed, even if the child is [their] own. [They were] told that other children were witnesses as well which is also unacceptable. [They] agreed and apologized. Inspection: Unannounced Monitoring.
