Staff person #1 has not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): PA Required Health and Safety for Family Childcare Part 1 and 2. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, a family child care home staff person who has completed all preservice trainings and has all qualifications to care for children unsupervised. The facility did not have an emergency plan on file. Staff 1 also did not have training on the emergency plan. Please see LIS for date of hire. Requirement: The facility shall have an emergency plan. The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training. Toxics were accessible under the kitchen sink. Various cleaners and toxics, such as pledge, were under there. The cabinet that the toxics were in was unlocked. Toxic plants were also observed by the bathroom that the children use. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. The rest equipment was not labeled with the children's names. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. Children 1, 2, 3 and 4 did not have the services provided section filled out. Children 1, 2 and 4 did not have a child services report completed. During the renewal inspection on 2/11/2026, it was observed that fire alarm testing was last done on 1/10/2026, making it more than 30 days since the last fire alarm testing. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: Alarms were tested. The facility did not have a method to contact parents when an emergency arises. The facility also did not have a method to contact parents when an emergency ended and how to reunite with their children. Requirement: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. Infant sleep position (3290.118)

During a verification visit on 930/2024. certification representative observed child 1 sleeping in a bouncer with a blanket. Provider did not state that there was a medical reason on file. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. It was also observed that the alarm on the third floor was operational but not interconnected with the alarms in the basement, first floor and second floor.
