Consent for administration of minor first-aid required prior to admission (3280.182(5))

At the time of the renewal inspection the certification representative observed that child #2 did not have signed parental consent for administration of minor first- aid procedures by facility staff. Requirement: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Correction: The parent will sign the administration of minor first-aid procedures by facility staff, consent on the bottom of the emergency contact form. Inspection: Renewal. At the time of the renewal inspection the certification representative observed that staff #2 and 3 had a health assessment that was more than 24 months. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. At the time of the renewal inspection the certification representative observed that the emergency contact information for child #1, 4, and 5 did not include the address and of the individual designated by the parent to whom the child may be released. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Pertinent Laws & Regulations/General Health and Safety (3280.14/3280.20)

At the time of the renewal inspection the certification representative observed that staff #1 did not have the verification of the completion of the 1-hour Health and Safety update. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: Staff #1 will provide documentation of the completion of the "1-hour health and safety update", "Getting Started in Center Based Care- Revised 2022" or they must complete PA Required Health and Safety for Centers-Group Child Care Parts 1 and 2. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. On 8/26/25 a certification representative conducted a complaint investigation. During the investigation, staff #1 and #2 reported that on 8/18/25 a child eloped from the school yard, while off school premises. The child had been away from facility staff for about 5-10 minutes before being located by staff #1 a few blocks away from the school yard. Staff #1 reported becoming aware of the missing child within seconds of them leaving the school yard. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in their assigned group. The staff person shall be physically present with the children in their group on the facility premises and on facility excursions off the facility premises. Correction: 1- 8/26/2025- Children will be supervised at all times. 2- 10/30/2025- All staff will receive a minimum of two hours of training in supervision. The training will be scheduled upon a referral by the Regional Office/Certification Representative. 3- 12/23/2025- The legal entity will begin using a checklist to count children as soon as the form is received that has been provided by the regional office/certification representative. The legal entity shall do this checklist for a minimum of three months from the date the checklist is received and will send it to the Regional Office weekly. Inspection: Complaints- Legal Location. At the time of the renewal inspection the certification representative observed that the facility did not have a written and posted plan which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency Requirement: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. Correction: The provider will develop a written plan and post in a conspicuous location, which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. At the time of the renewal inspection the certification representative observed that the provider did not notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the child care facility. Requirement: The operator shall notify local traffic safety authorities in writing of the location of the facility and about the program's use of pedestrian and vehicular routes around the group child care facility. Correction: The provider will submit to the local traffic authority, the facility's written plan identifying the location of the facility and the program's use of pedestrian and vehicular routes around the childcare facility.
