A renewal inspection was conducted on 1/29/2026. At that time, the facility's fire detection testing log was reviewed. The log indicated that smoke detectors were tested on 12/10/2025 and again on 1/12/2026, which exceeds the required 30-day testing interval. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: The fire detection testing log is current. In the future, we will test the alarms every 30 days and document the testing on the facility's fire detection testing log. Evacuation routes posted (3290.94(a)(5))

A renewal inspection was conducted on 1/29/2026. At that time, facility did not have evacuation routes posted in the basement or on the second floor of the facility. Requirement: Evacuation routes are posted in a conspicuous location on each floor of the facility. Correction: At the time of the inspection, evacuation route postings had been temporarily removed in order to place them in the facility compliance folder for review by the inspector. The evacuation routes were previously posted in all required locations. Immediately during the inspection visit, evacuation route postings were returned properly displayed in the locations of each floor of the home. Inspection: Renewal. Written emergency plan posted (3290.124(d))

A renewal inspection was conducted on 1/29/2026. At that time, the facility's emergency transportation plan was not posted within the facility and was not present in the vehicle used to provide transportation. Requirement: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children. Correction: At the time of inspection, the facility's written emergency transpiration plan was not present inside the vehicle used for transportation or posted in the child care space. Immediate corrective action was taken during the inspection, and the written emergency plan was placed inside the glove compartment to ensure accessibility at all times. The emergency transportation plan was also posted in the child care space. The emergency transportation plan is now maintained in the vehicle and will remain readily available whenever children are transported. Vet's certificate required (3290.117(c))

A renewal inspection was conducted on 1/29/2026. At that time, rabies vaccine on file for the dog at the facility expired 10/5/2025. Requirement: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present. Correction: At the time of inspection, the rabies vaccination certificate for the home dog had expired. Immediate corrective action was taken. An appointment with the veterinarian was scheduled the same day as the inspection was 1/29/2026, and the dog was seen by the veterinarian on 1/30/2026, where the required vaccination for rabies immunization were updated. Comply with CPSL/CPSL information (3290.32(a)/3290.192(3))

A renewal inspection was conducted on 1/29/2026. At that time, staff and household member files were reviewed. The file for Facility Person #2 contained a PSP clearance dated 9/22/2020 and a PSP volunteer clearance dated 2/4/2025. A volunteer clearance is not an acceptable clearance. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #2 shall not be present in the home while children are in care until acceptable clearance is on file. Correction: At the time of the inspection, a volunteer clearance was already on hand for Facility Person #2, and other clearances for a local private school (St John Vianney). During the visit it was stated that the required clearance not to be solely volunteer and it was immediately updated to ensure full compliance with CPSL and Chapter 3490 regulations. Age and training/Age and training (3290.31(f)(4)(i)/3290.32(d))

A renewal inspection was conducted on 1/29/2026. At that time, staff and household members files were reviewed. Facility Person #1 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring 9/30/2025. Facility Person #1 completed the online portion of a pediatric first-aid/CPR course; however, a complete certification, including the required in-person skills assessment, was not on file. Documentation of updated and complete Pediatric first aid and CPR training is not on file. Facility Person #1 is the sole provider of child care at the facility. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required training has been completed, Facility Person #1 must be supervised, when interacting with children at a minimum by, a r family child care home staff person who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise Facility Person #1, Facility Person #1 may not work in a child-care position at the facility. One or more facility persons competent in pediatric first-aid and pediatric cardiopulmonary resuscitation techniques shall be at the facility when one or more children are in care. Correction: Due to severe winter weather conditions and multiple weather-related cancellation of their in-person classes, I was unable to complete the required in-person Pediatric First Aid and CPR skills assessment prior to the inspection date, which delayed completion despite efforts to obtain certification in a timely manner. Upon notification, immediate correction was made and the class was rescheduled for (2) days later, a plan of correction was implemented, and the required in-person training was completed. Clean and good repair (3290.102(a))

There is a ripped pillow with exposed foam in the childcare space. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
