During the renewal inspection on 3/6/2026, it was observed that the emergency plan drill that was on file was dated 2/3/2025 making it more than 1 year since the last drill. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: Staff will do an emergency drill on Monday. Emergency info/agreement updated 6 mos (3270.181(c))

Child 3 had outdated emergency form and fee agreements on file during the renewal inspection on 3/6/2026. They were last updated on 7/22/2025 making it more than 6 months since the last update. Requirement: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Child's hands washed (3270.134(a))

Staff 1 did perform a diaper change during the renewal inspection. Staff 1 did not wash the child's hands after diapering. Staff 1 did express concerns about being able to appropriately supervise the children during handwashing due to the sink being out of sight of the children and she was by herself with the children. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Legal entity will develop a handwashing policy and review the policy will all staff. Correction: I will develop a handwashing policy. Inspection: Renewal. Services proceeded (3270.123(a)(3))

Child 2 did not have the services provided section filled out on the fee agreement. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Age and Training (3270.31(e))

Staff 4 had 4 hours of annual training on file. Staff 5 had 8 hours of annual training on file. Staff 4 and 5 did not meet the annual 12 hours of childcare training requirement. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Clean, good repair, proper size (3270.104(a))

In the caterpillar room, the diaper pad was ripped, making it unable to be properly cleaned and sanitized. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: Ripped diaper pad will be ordered and replaced immediately. Building Surfaces (3270.76)

The outside play area had a loose board on the wooden fence. The board also had a rusty nail sticking out of it. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Staff evaluations (3270.34(a)(6))

During the annual inspection on 3/3/2025, staff 8 and 9 did not have a staff evaluation on file. Staff 1, 2, 4, 5, 6 and 7 had outdated staff evaluations. Staff 1's staff evaluation was last completed on 2/29/2024. Staff 2's staff evaluation was last completed on 12/15/2023. Staff 4's last staff evaluation was completed on 7/30/2023. Staff 5's last staff evaluation was completed on 1/22/2025. Staff 6's last staff evaluation was completed on 1/20/2024. Staff 7's last staff evaluation was completed on 12/13/2023. Please see LIS for date of hire. Requirement: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months. Fire safety - 1 yr. (3270.31(e)(4)(ii))

During the annual inspection on 3/3/3025, staff 1, 2, 3, 4, 5, 6, 7, 8 and 9 ad outdated fire safety training. Fire safety training for these staff occurred on 2/15/2024 making it more than 12 months since the last fire safety training. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Training regarding plan (3270.27(c))

Staff 1, 2, 4, 5, 6,7, 8, 9 and 10 had outdated emergency plan training. The emergency plan training for these staff was last given on 2/27/2024, which was more than 12 months ago. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.
