Plan reviewed/updated annually/Training regarding plan (3270.27(b)/3270.27(c))

During a renewal inspection on 2/23/26, there was no written documentation of the annual plan review since January 2025. Staff # 1 last reviewed the emergency plan on 1/10/25. Requirement: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility. The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Correction: The operator will document an emergency plan review. The director will advise staff of any changes and have them sign to acknowledge the review. Inspection: Renewal. Refrigerator (3270.107)

During a renewal inspection on 2/23/26, certification rep observed the thermometer in the refrigerator in the older toddler classroom was 50 degrees. This refrigerator is used to store the lunches belonging to the older toddler children. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The director adjusted the temperature of the refrigerator and placed another thermometer inside to determine if the refrigerator or thermometer is faulty. Infant: updated health report every 6 months (3270.131(b)(1))

During a renewal inspection, more than 6 months passed between health reports for children # 1, 2, and 3 (see code sheet for DOBs). Child # 1, a young toddler, has health reports dated 2/13/25 and 10/13/25. Child # 2, a young toddler, has health reports dated 1/28/25 and 12/9/26 and child # 3, a young toddler at the time, has health reports dated 1/22/25 and 12/18/25. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Designated release persons/Admission date (3270.123(a)(5)/3270.123(a)(6))

During a renewal inspection on 4/30/25, there was no release persons or date of admission on the agreements for children # 1, 2, 3, 4, and 5. Requirement: An agreement shall specify the persons designated by a parent to whom the child may be released. An agreement shall specify the date of the child's admission. Correction: Agreements were corrected the day after inspection and scanned to inspector. All the proper information was input Inspection: Renewal. Exam communicable disease/Physician/CRNP assessment (3270.151(c)(3)/3270.151(c)(5))

During a renewal inspection on 4/30/25, the health assessment on file for staff # 1 did not include an examination for communicable diseases and results or the physician's assessment of their suitability to provide child care. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care. Correction: Director contacted Medicus and had all the information added to the form, Director also had Medicus fill out the form suggested by the state. Inspection: Renewal.
