During a complaint investigation conducted on 8/7/2025, The investigation found that the child put her fingers between the door and door frame. This action caused severe injury to the child's finger. The child was treated in the emergency room for a broken finger. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Children will gather on the opposite side of the door to prevent future incidents.
