Staff person #1 has not completed the following pre- service training required prior to caring for children unsupervised: Mandated reporter training and Health and Safety Training. Based on interviews, Staff #1 and #2 transported 7 preschool children in an 8-passenger van. Based on interviews, Staff #1 and #2 indicated they had 6 booster seats so they placed two children in one booster seat with one child positioned in front of the other child. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: Staff will not accept children that they do not have room for in the van. Staff #1, a provisional hire, lacked a National Sex Offender Registry Certificate (NSOR) prior to providing initial service in a child care setting (SEE LIS Code Sheet). Certification Staff observed Staff #1, a provisional hire, working alone in the Infant Room and not in the vicinity of a permanent employee. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 1 may not work in a child care position at the facility. A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. On 1/13/26 Staff #4 transported 2 preschoolers without a second staff person needed to meet the requirement of staff-child ratio. The facility was observed to have trash cans that are used to collect trash that has been contaminated by human secretions or excrement that lacked plastic-liners. The first-aid kit in the facilities large muscle group space was observed to contain a first-aid kit that lacked disposable nonporous gloves. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Building Surfaces (3270.76)

Uneaten peperoni slices were observed lying on the floor in the school aged room. Handwashing signs (3270.82(h))

The sink in the facility's 1's room was observed to lack a handwashing sign. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Clean and good repair (3270.102(a))

The facility was observed to have tape on the edge of the bookshelves that was partially peeled away, and a yellow table that had loose legs, in the school aged room. In the 2's room, a wooden vanity table was easily tipped, and a wooden table was observed to be unsteady from loose legs. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Tape was removed from edges of bookshelves, the yellow tables legs were tightened, the wooden tables legs were tightened and made even, the wooden vanity was screwed to the wall. Small Toys and Objects (3270.103)

A dime was observed lying on the floor in the 1's room. A plastic Walmart bag stuffed full of disposable plastic shopping bags was observed accessible to children in the 2's room. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Dime was removed from floor in the one-year-old room. The plastic bags were removed from the 2-year-old room. Inspection: Renewal.
