The following was self-reported non-compliance: According to staff written and verbal statements, Staff person #1 shoved a shelving unit toward Child #1 in an attempt to stop the child from running, hitting Child #1 in the face with the shelf during the incident. This resulted in an injury to the child. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Staff person #1 was immediately removed from the classroom upon notification of the incident, and placed on administrative leave. There was an unlidded trashcan in Room 205 that contained empty individual milk containers and wrappers that children had thrown away after snack. There was an unlidded trashcan in Room 207 that contained soiled facial tissues. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: Trash cans with lids were added to both classrooms. The current emergency contact form on file for Child #2 does not indicate if the child has any special medical needs, which is needed in an emergency situation. Requirement: Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant or CRNP, which is needed in an emergency situation. Correction: Form spaces were left blank due to Child #2 not having any special medical needs, however, the spaces have now been crossed out and N/A added.
