In the beginning of March 2026, Staff person #1 admitted they took pictures of child #1 on the toilet. The staff person sent the picture to child's parent to show how child was progressing with potty training, Parent did not give permission or requested a picture of child on the toilet. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Correction: A staff meeting was held and discussed about the importance of privacy toward all ages of children. Phones or any devices that take pictures are not allowed in the bathroom. Health and Safety Training (3270.31(f))

Staff person #5 has not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): (Pediatric First Aid and CPR). Staff person #6 has not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): (Pediatric First Aid and CPR). Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Until such time as the required training has been completed, staff person #5 and staff person #6 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #5 and staff person #6, staff person #5 and staff person #6 may not work in a child-care position at the facility. It was observed in the school-age classroom, Clorox wipes and a cleaning spray were placed on the counter accessible to children. It was observed in the pre-k classroom, White Out that indicates "keep out of reach of children" was stored in a drawer of the teacher's desk that was not locked or made inaccessible to children. It was observed in the older toddler classroom, White Out that indicates "keep out of reach of children" was stored in a drawer of the teacher's desk that was not locked or made inaccessible to children. It was observed in the back toddler classroom, a Magic Eraser was not stored in an original container or in a container that specifics the content. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The magic eraser was placed in its original box in the maintenance room which is out of the reach of children and is a locked room. Contaminated trash in closed plastic-lined receptacle (3270.67(d))

It was observed that a closed, plastic-lined trash can was not present in the front infant classroom, back infant classroom, front toddler classroom and back toddler classroom. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Correction: We have added closed and plastic lined trash cans in both infant classrooms and both toddler class rooms. Inspection: Renewal. Building Surfaces (3270.76)

It was observed in the older toddler, preschool, and pre-k classes a buildup of dust, dirt and debris on the toy shelves. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. It was observed in the back toddler classroom, no platform or step at the sink was available to children for handwashing. Requirement: Toilets and sinks shall be at proper heights for children using them or shall be easily approached by means of platforms or steps. It was observed in the school-age bathroom, pre-k bathroom, preschool bathroom, and older toddler bathroom that handwashing signs were not posted at each toilet and diapering areas. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. It was observed in the preschool classroom nap mats being used by children were not at least 2 feet apart on three sides. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: We have mapped out a sleep map to make sure all the children are always at least 2 feet apart on all three sides at all times during nap. Inspection: Renewal. It was observed in the front infant classroom, school-age classroom, front toddler classroom and back toddler classroom that a written plan of daily activities was not posted in each space. Supervised at all times (3270.113(a))

It was observed in the preschool classroom, music being used during rest time at high volume level that could potentially prevent staff from being able to properly hear, direct and assess children while sleeping on nap mats. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: The volume of the music during sleep time was turned down immediately. The files for children #1, #2, #3 and #4 did not include child service reports conducted within the last 6 months. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). It was observed in the back toddler classroom that staff did not wash a child's hands after being diapered. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: It was discussed to all staff in the Toddler room and all rooms, the importance of hand washing after diapering and using the bathroom. It was observed in the front toddler classroom and back toddler classroom that staff were not appropriately utilizing the hands-free trash can by keeping the lid in an open position. It was observed in the pre-k classroom that diapers were not being disposed of into a hands-free covered trash can. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: We have gotten hands free trash cans in both of the Toddler class rooms and in the pre-k class room for the soiled diapers. Inspection: Renewal. File for staff person #1 did not complete a health assessment at least 24 months after the previous health assessment. The current completed health assessment is dated 8/27/25 and the previous health assessment is dated 6/23/23. File for staff person #2 did not complete a health assessment at least 24 months after the previous health assessment. The current completed health assessment is dated 11/28/25 and the previous health assessment is dated 2/20/22. The file for staff person #3 does not include a completed health assessment at least 24 months after the previous health assessment dated 10/11/23. The file for staff person #4 does not include a completed health assessment at least 24 months after the previous health assessment dated 9/28/23. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Correction: We have come up with a schedule to review all staff files to make sure all clearances and files are up to date. Fire drills have not been conducted at least once every 60 days. Fire drills were documented to have occurred 9/5/25-11/7/25. Requirement: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days. Correction: The next fire drill will take place in January 9, 26. We have got a written schedule when the 60 days are to be done and we will move 2 days before that Inspection: Renewal. The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. It was observed in the young toddler and older toddler classrooms that there is not a variety of play materials to promote the children's developmental needs and individual interests. Requirement: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use. Plastic grocery bags were observed in the young toddler classroom accessible to children. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: Plastic bags were removed and have been made inaccessible to children in young toddler classroom Inspection: Unannounced Monitoring. Correction: I have written down future dates of the fire detection devices to make sure they are tested at least once every 30 days. Supervised at all times /Staff assigned to specific children (3270.113(a)/3270.113(a)(1))

During the investigation it was stated by staff person #2 and staff person #3 that child #2 was found sitting alone in a supply closet left unsupervised by primary staff person #4. It was also stated by staff person #2 and staff person #3 that they had closed the closet door leaving the child unsupervised. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The legal entity must arrange for staff persons #2, #3, and #4 to complete supervision training through Better Kid Care; Supervision: What's Required. Correction: We had a meeting with staff 2,3,4 the importance of making sure that you are aware of your children and counting the children in your group. I also stressed to staff 2 and 3 that the director should have been notified of the incident. Staff 2,3,4 will complete required supervision training. Inspection: Complaints- Legal Location. No physical punishment (3270.113(b))

During the investigation it was stated by two witnessing staff members, that staff person #1 physically grabbed and picked child #1 up by the wrist from the floor, drug the child #1 across the floor and forcefully placed child #1 in a chair. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Up on this investigation Staff per 1 has been warned numerous times about using physical force on children.
