Observed the facility did not have a supervision policy during the transfer of care for the school-age children who are picked up and dropped off by the school bus. Requirement: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. Clean and good repair (3270.102(a))

Observed a white plastic shelf containing food was not securely anchored to the wall and could be pulled over by a young toddler in the room. Observed a broken clear toy bin located in the preschool eating/play area. Observed a damaged/broken plastic bowling ball pin in the room across from the old infant room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Observed in the old infant room, several spots of damaged and peeling paint under the first aid kit. Contaminated trash in closed plastic-lined receptacle (3270.67(d))

Observed in the first room on the left room 101, there was a trash can which did not have a lid and contained used paper towels and tissues. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Locked or inaccessible (3270.66(a))

Observed paints, labeled "keep out of the reach of children", accessible to children in an unlocked storage room next to the back toddler room, due to the door not shutting completely. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The door was secured and a sign was placed on the door to remind staff to keep the door closed at all times. Inspection: Renewal. Protective Outlet Covers 5 yrs. or less (3270.65)

Observed an electrical outlet which did not have protective receptacle covers, under a highchair in the back toddler room. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: Outlet cover was placed in the electrical outlet. Observed staff files #2 and #3 did not contain documentation of training in the emergency plan at the time the emergency plan was updated. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. Observed the immunization record for child file #5 was not complete (updated) and the file for child #5 did not contain a letter of exemption from immunizations to document why the immunization record was not complete. Requirement: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Stairs, exits, etc. unobstructed (3270.91(a))

Observed a front hallway blocked with excess toys and equipment, a back exit door blocked with a broom, and another back exit blocked by a table and chairs. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Observed cheese and danimals (yogurt) which were both labeled keep refrigerated, on a low counter in the toddler eating area and were not refrigerated as required. Emergency plan (3270.27(f))

Observed the facility had an incomplete emergency plan and because it was incomplete the facility did not send a copy of the emergency plan to the local municipality and to the county emergency management agency. Requirement: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency. Observed the facility had applied for a new occupancy permit but did not have a copy of the occupancy permit on file at the time of the inspection. Requirement: A certificate of compliance will not be granted by the Department until the legal entity provides a certificate of occupancy as proof of compliance with the applicable requirements of the Department of Labor and Industry in 34 Pa. Code § 403.23 (relating to child day care facilities) and under section 1016(c) of the act (62 P.S. § 1016(c)).
