Then, staff person #4 walked into the room from the back hallway to supervise the children. On 8-14-25, staff person #6 and #7 were observed on an excursion leaving the nearby high school with 18 school-age and preschool children and were walking back to the facility. Staff person #6 and #7 both stated that they were sharing the group of 18 children, with staff person #6 naming all 18 children, and staff person #7 naming 16 of the 18 children and stated that they did not know 2 of the children's names. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. On the outdoor play space, there were 4 bolts on the inside of the fence near the gate and right corner that were protruding out and were a visible hazard. In the upstairs back stairwell, the windowsill had cobwebs and visible dirt. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The four protruding bolts were safely capped with protective coverings. The windowsill was thoroughly cleaned on 6-11-25 with cobwebs and dirt removed. Inspection: Renewal.
