During the renewal inspection on 1/5/2026, it was observed that 11 out of 14 of the Emergency Contact Forms in the Sharks room were not updated within a 6-month period, and in the After-school room 21 out of 27 Emergency Contact Forms were not updated. During the renewal inspection on 1/5/2026, the file for child #1 was viewed and the health assessment was dated 12/31/2024 which was over 12 months. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. During the renewal inspection on 1/5/2026, it was observed that the cover of the changing pad in the care Bears room and the Butterfly room were splitting at the seams. In room #11 the laminate on the bookcase was peeling off. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. The expiration dates of the medication for child #2 was 1/2022, for child #3 was 7/2024, for child #4 was 3/2024, for child #5 and child #6 the date was 1/2024, and for child #7 the expiration date was 2/2022. Requirement: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label. During the renewal inspection on 1/16/2025, it was observed that the changing pad in the infant room had tears in it, the covering on the potty seat from room #6 was ripped, the play couch in room #8 was split at the seams, in room #15 and room #7b there was a pair of sharp scissors on the cabinet and they were accessible to the children and the top of the cubby outside the room was dirty and needed to be cleaned, and room #11 had floor mats that were dirty and needed to be cleaned. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The infant changing table pad and the potty seat cover will be replaced, the play couch will be patched up, the scissors were put up on a high shelf, the cubby will be cleaned and wiped down, and the floor mats will be thrown away.
