Staff #1 lacked a written evaluation within a minimum of 12 months of employment. (SEE LIS CODE SHEET) Requirement: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months. Correction: The staff evaluation was late due to the change in Executive Director and will be completed by new Executive Director and placed in the staff file. Inspection: Renewal. The PA State Police, Child Abuse and FBI Clearances in the files for staff #2 were dated more than 60 months apart. (SEE LIS CODE SHEET) Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Free from contagious/communicable disease (3270.131(d)(7))

The Child Health Report for Child #1 lacked if the child is able to participate in childcare and free from contagious/communicable disease. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Correction: I spoke with the parents of Adalee and gave parents a new form and one week from the date to provide an updated health form.The parent corrected the form within 24hrs. 11-6-24 Inspection: Renewal. Name/address/phone release person (3270.124(b)(7))

The Emergency Contact form for child file #1 lacked the address of the listed release person. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: I spoke with the parent regarding adding an address to emergency contacts to those whom the child could be released to and she was able to correct it the same day.11.5.24. General Health and Safety (3270.21)

In Room 203R, three (3) pair of adult scissors which were located above the sink and were accessible to children posed a cutting hazard. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children.
