The file for Staff Person #1 had NSOR Certificates dated more than 60 months apart (specifically: 2/13/2020 - 2/19/2025). The file for Facility Person #2, a volunteer, contained PA State Police Clearances (6/11/2020 - 11/13/2025) and PA Child Abuse Clearances (6/12/2020 - 11/19/2025) dated more than 60 months apart. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: The Staff Person #1 has an NSOR in the file and was 7 days late. Age and Training (3270.31(e)(4)(i))

The two most recent Pediatric First Aid/CPR trainings for Staff Person #1 were dated more than 24-months apart (11/15/2023 - 11/19/2025). Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: The first aid certificate is placed in the Staff's file. It iis renewed, but was l4 days late. It is in the file and will be renewed by the expiration date. Inspection: Renewal. Protective Outlet Covers 5 yrs. or less (3270.65)

In the upstairs Pre-School room near the sleep supplies, an outlet was observed to be missing one of its protective covers. In the Gym along the striped wall on the left, an outlet was observed to be missing one of its protective covers. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Locked or inaccessible (3270.66(a))

In the back area of the Young Toddlers room, the cabinet beneath the changing table was left unlocked. The cabinet contained ointments and creams marked "keep out of reach of children." In the School Age room, a bag of sidewalk salt/ice melt labeled "keep out of reach of children" was being stored near the exit. The bag was left open and in reach of children. In the School Age room, a stick of deodorant, which is considered toxic, was stored in an unlocked drawer near the indoor play space. In the walkway/entry area between the downstairs Preschool and School Age rooms, an open jug of pressure washing solution (marked keep out of reach of children) was being stored in the open, near the entry door. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The cabinet in YT room (back) has been locked. School age room bag of salt is in a locked cabinet. School age room stick of deodorant was thrown out and will not return. Building Surfaces (3270.76)

In the Young Toddlers room (front), there was splintering wood observed on the bottom of the entry door (surface facing the inside of the classroom). In the same room, significant dirt buildup was observed in the corners of the floor near the kitchen area. In the downstairs Preschool Room, rust was observed in the threshold of the doorway leading to the outdoor area. In the same room, a water stain was observed on one of the ceiling tiles above the door to the maintenance room. The countertop area by the handwashing sink in the downstairs Preschool room had a sharp edge to it that poses a hazard to children. In the kitchen area of the downstairs Preschool room, significant dirt buildup was observed on the floor next to the refrigerator. In the School Age room, significant dirt and grime buildup was observed on the faucets in both the boys' and girls' restrooms. In the School Age boys' restroom, to the right of the cabinet near the sink, significant dirt buildup was observed on the floor. The Outdoor Play Space includes a rubber walkway in the upper play area. One area of that walkway was missing a portion of rubber approximately 3 inches in diameter, leaving the surface in poor repair. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: 11/22: All splinters [on the] door on the bottom has been repaired. The dirt has been removed from near the kitchen area in both rooms. Water-stained ceiling tile has been painted and rust has been removed. SA room - the faucets were cleaned and next to the sink in boys room on floor. Outdoor play space will be repaired 12/19. No peeling paint or plaster (3270.77(a))

In the Young Toddler room (back), damaged/flaking plaster was observed on the back wall to the left of the last window on the right. Additional flaking plaster was observed near the floor to the right of the children's cubbies. In the upstairs Preschool room, multiple areas of damaged plaster and peeling paint were observed on the green wall to the right of the kitchen area, below the windows. In the downstairs Preschool room, damaged plaster was observed on the rear wall beneath the "preschool class" sign. An additional area of damaged plaster was observed to the right of the height chart at the back of the room (near the floor). In the Gym, on the wall to the left by the pads, an area was observed to have cracked and peeling plaster near the trim. On the same side of the room, multiple areas of peeling paint were observed on the striped section of wall. In the Outdoor Play Space, the following items were observed to have areas of peeling paint: the green picnic bench, the red table and the small green bench. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Handwashing signs (3270.82(h))

There was no handwashing sign at the sink in the boys' bathroom area of the School Age room. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Clean and good repair (3270.102(a))

In the Young Toddler room (front), a gray bin and a green bin were both observed to contain wooden blocks that showed evidence of being chewed. These blocks were splintering, and the paint was peeling, creating a hazard to children. In the Young Toddler room (back), a clear bin was observed to contain wooden blocks which were splintering in places. The splintered blocks pose a hazard to children. In the upstairs Preschool room, a shelf in the play space contained a crate with foam blocks. The foam blocks had teeth marks on them and were missing foam in places. In the same area, a bin of wooden number blocks (marked Lakeshore) contained several which were in poor repair. Specifically, the backs were separating from several of the blocks which contained small beads. These pose a hazard to children. Also in the upstairs Preschool room, a Home Depot play workbench in the rear play area was observed to be in poor repair. The play item was wobbly and unstable, creating a hazard to children. Also in the same area, a pink toy changing table was observed to have a hole in the top that measured approximately two inches. The hole created sharp edges, which could injure children. In the downstairs Preschool room, a bin of foam puzzle pieces was observed on one of the shelves in the back of the room near the yellow table. The puzzle pieces contained teeth marks and had areas of missing foam. In the School Age room, a bin of wooden blocks was observed on a shelf near the stage. The blocks were observed to be splintering, creating sharp edges. In the Gym, the ball pit was observed to contain several balls which were damaged. The observed damage included bite marks, and some of the damaged balls were folded in on themselves, creating sharp edges. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Small Toys and Objects (3270.103)

In the Young Toddler room (front), a gray bin and a green bin were both observed to contain wooden blocks that were splintering, creating a choking hazard to children still placing objects in their mouths. In the Young Toddler room (back), a clear bin was observed to contain wooden blocks which were splintering in places. The splintered areas create a choking hazard to children still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Services proceeded (3270.123(a)(3))

The file for Child #1 (SEE LIS CODE SHEET) was lacking a Child Service Report. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: A child service report has been placed in the childs file (#1child) and will remain in the file and all future service reports will be in a childs file. Inspection: Renewal. Immunization record (3270.131(d)(5))

The file for Child #2 included a Health Assessment; however, the health assessment did not include a review of the child's immunization records. (SEE LIS CODE SHEET) Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. ACIP recommended immunization record (3270.131(e))

The records for Child #3 and Child #4 both included health assessments with written verification of immunizations; however, those documents were dated more than 60 days after the admission of the children. The record for Child #2 included a health assessment that did not have written verification of their immunizations, and the child has been attending the facility for more than 60 days. (SEE LIS CODE SHEET) Requirement: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Correction: Children will have timely health assessments provided by the parent or will be removed from the facility until one is obtained. Regarding Child #2, parents will provide immunization records. Child will be removed until immunization records are obtained. Inspection: Renewal. Insured (3270.176(a))

Vehicle #2 did not contain a valid insurance card. During inspection, the operator could not locate documentation of insurance for Vehicle #2. Requirement: A vehicle shall be insured in accordance with the requirements of 75 Pa.C.S. § § 1701-1799.7 (relating to the Motor Vehicle Financial Responsibility Law). Clean, good repair, proper size (3270.104(a))

In the infant room, a black desk chair was observed to have areas of flaking leather/vinyl. In the Young Toddler room (back), a wooden shelf was observed on the back wall (under the window nearest the kitchen). The shelf was splintering on the top surface, creating a sharp edge. In the upstairs Preschool room, a child-sized red and blue vinyl chair was observed in the rear play area. The chair was in disrepair, with blue vinyl flaking from the bottom panel. In the same room, a set of white plastic drawers was observed to the left of the children's cubbies. The drawers were broken and as a result were leaning/wobbling to the left. In the downstairs Preschool room, flaking rust was observed on a metal cabinet near the rear entrance. In the School Age room near the exit to the outdoor play area, a wooden bookshelf was observed to have a long splintered section on the bottom shelf. In the Outdoor Play Space, the following items were observed to be in poor repair: the green picnic bench, the red table and the small green bench. All three items were observed to have areas of peeling paint which had flaking rust underneath. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: The chair from the infant room has been tossed into the trash. In the young toddler room, the wooden shelf was repaired. A new chair has been bought for the preschool room and will remain in the classroom. The old chair was removed. Inspection: Renewal. During the complaint investigation, Staff Person #1 admitted to having placed the child's name on a dry erase board in the classroom, along with a number, due to behavioral issues. Staff Person #1 reported that it was a reminder to speak with the parent about the child's behavior at pickup. The dry erase board was in view of anyone who would enter the classroom. Requirement: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family. Correction: Name and number has been removed months ago. Also was not in use on day of inspection. The teacher is retired and no other teacher will use this form of calculation. Similar Age Level (3270.51)

Based on staff statements, the facility is occasionally out of ratio in the morning between 7:00 am and 7:30 am when children and staff are arriving. Requirement: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. Correction: Moving forward to ensure ratio is maintained when children arrive and are in a group together the group will divide with a teacher according to the following: Infant 1:4 & maximum of 2:8; Y Toddlers 1:5 & maximum of 2:10; O Toddlers 1:6 & maximum of 2:12; Pres 1:10 & maximum 2:20; YSA 1:12 & maximum 2:24. Mixed Age Level (3270.52)

Based on staff statements, the facility is occasionally out of ratio in the morning between 7:00 am and 7:30 am when children and staff are arriving. On 9/2/25 at 7:30 am in the younger toddler room, Staff # 2 and #3 were observed caring for 15 children, 3 Infants, 3 Young Toddlers, 6 Older Toddlers and 3 Preschool aged. Staff #1 had been in the younger toddler room with the other two staff, prior to leaving the room to answer the door. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Staff assigned to specific children (3270.113(a)(1))

On 9/2/25 at 7:30 am in the Infant room, Staff # 2 and #3 were observed caring for 15 children, 3 Infants, 3 Young Toddlers, 6 Older Toddlers and 3 Preschool aged. Staff #1 had been in the Infant room with the other two staff, prior to leaving the room to answer the door. When staff #1 returned to the room, all three staff were asked to identify the children they were responsible for supervising. Staff #1 identified 3 Infants and 1 Young Toddler, Staff #2 identified 3 Preschoolers, Staff #3 identified 2 Young Toddlers and 2 Older Toddlers. 4 Older Toddlers were not identified. When asked which staff was responsible for the remaining 4 Older Toddlers, staff stated that Staff #6, who was in ratio in another classroom. CR explained that all of the children present needed to be assigned to a staff in the same child care space. Staff #2 added the 4 children to their group. On 9/2/25 at 7:40 am, in the Downstairs Room #1, School Age, Saff #4 and Staff #5 were observed caring for 13 children, 2 Preschoolers, 6 Young School Age and 5 Older School Age. When asked to identify the children each staff was responsible for supervising, Staff #4 identified 1 Preschooler and Staff #5 identified 5 Young School Age and 5 Older School Age. 1 Preschooler and 1 Young School Age child were not included in either staff's group. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: All groups will remain separated and enforced to carry identification cards of the children. Each time a child enters the classroom the teacher will retrieve the child's identification card. So they will have a designated group count each time a child arrives. The upper portion of a child size art paper dispenser, observed in the main floor PS room, was not secured to the base creating pinch points. A broken yogurt container was observed in the sand table in the school age room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The paper dispenser was removed from the classroom and is disposed. The yogurt cup was removed by staff while the certification representative was present. Inspection: Renewal. Facility persons #1 and #4 did not complete mandated reporter training within 90 days of hire. Both volunteers completed the training in 10/2024. This is evidenced by the documented mandated reporter trainings on file for staff #2 being dated 9/04/19 and 9/25/24, and the trainings for staff #3 being dated 5/07/19 and 9/26/24. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: All volunteers and employees will complete the mandated reporting within 90 days of the start date from this date forward. Inspection: Renewal. The file for staff #2 contained clearances which were not renewed within 60 months; the documents were dated as follows; SP 8/27/19 renewed on 9/12/24, CA 9/10/19 renewed on 9/30/24, FBI 8/30/19 renewed on 9/13/24. The file for staff #3 contained clearances which were not renewed within 60 months; the documents were dated as follows; SP 5/28/19 renewed on 9/26/24, CA 5/24/19 renewed on 10/12/24, FBI 5/28/19 renewed on 10/18/24. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility. Correction: The facility persons record shall include a copy of the proper DHS FBI Clearance. Hand sanitizer, labeled keep out of reach of children, was accessible to children in the rear older toddler care space. The hand sanitizer was observed in a side pocket of an adult's personal bag which was sitting on a low shelf. Mutiple containers of cleaning materials were accessible to children in the school age room. The toxics were observed on the lowest shelf of an unlocked storage cabinet. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Hand sanitizer has been removed. Explained the regulations to the behavioral specialist and reminded staff that no hand sanitizer can be accessible to children.
