During the allocated unannounced inspection on 4/6/2026, it was observed that the facility's wooden fence in the outdoor play space had multiple broken boards and a nail sticking out. The broken boards could potentially cause a splinter if the children went up against the boards. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards. Correction: I will make the necessary repairs to the fence. Children will not use the yard in the meantime and will go for walks. Inspection: Allocated Unannounced Monitoring.
