During the renewal inspection conducted on 1/15/2026 observed the rubber baseboard molding in the bathroom was peeling off. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. During the unannounced monitoring inspection conducted on 8/6/2025 observed staff person #1's pediatric first aid and cardiopulmonary resuscitation (CPR) training was expired. The last training on file is dated 1/10/2023 and expired on 1/10/2025. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: Staff members all received Pediatric First Aid & CPR on August 16th, 2025, including the staff member listed above. All staff now have current Pediatric First Aid & CPR Training. During the renewal inspection conducted on 1/28/2025 observed child #1 and child #2's emergency contact information did not include the work addresses and telephone numbers of the enrolling parent. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. During the renewal inspection conducted on 1/28/2025 observed staff person #2's file did not include a copy of the disclosure statement. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
