Documentation shall be retained in the facility persons file in an electronic system as designated by the Department. During unannounced visit 06/26/2025, file of child #1 did not contain an incident report from Incident 06/04/2025 where child was treated at facility for wrist being red; incident reports on file for child #1 from 01/06/25 and 01/16/25 did not contain parent's signature. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: Incident report for child #1 was completed on July 2 with parent signature and original was given to the parent after making copies for child file and binder designated for all incident reports filed by individual classrooms. Additionally regulations and policies related to health and safety, including food related safety, will be reviewed with all staff at a mandatory staff meeting 06/13/2025. Staff will acknowledge their understanding of the regulations and policies by signing off at the completion of the training. Staff will be specifically instructed not to eat the children's food or give the children food that could be considered unsanitary. Finally, staff will be reminded to escalate any concerns they have regarding the behavior of other staff members to management and to report to ChildLine when necessary.
