Observed child #1 in the preschool room and there was not a copy of the child's emergency contact form present. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Pertinent Laws & Regulations/Communication with Parents (3280.14/3280.21)

Staff persons #1 and #2 had completed first aid and CPR training, however the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum and the trainer has not provided documentation that they are PQAS-certified in that training curriculum. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. The operator shall establish oral or written communication in the language or mode of communication which is understandable to the parent. Until such time as the required training has been completed, staff persons #1 and #2 must be supervised, when interacting with children at a minimum by, an (AGS, primary staff person, or family child care home staff person) who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff persons #1 and #2, staff persons #1 and #2 may not work in a child-care position at the facility. General Health and Safety (3280.20)

Observed child #1 not properly secured in a highchair based on the manufacturer's guidelines sue to the fact the shoulder straps present in the equipment were not in use. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: When a child is sitting in a high chair they need to have straps on and used properly. The day of my evaluation, the child was buckled in but we didn't use the shoulder straps. Clean and good repair (3280.102(a))

Observed a cracked lid on a bin of toys housed on a shelf outside the entrance to the playroom. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. FIRE DETECTION/Staff responsibility (3280.95(a)/3280.95(b))

Based on the smoke detection system testing documentation, testing was done more than 30 days apart on the following dates: 8-20-24 to 9-20-24 = 31 days; 9-20-24 to 10-22-24 = 32 days; 10-22-24 to 11-22-24 = 31 days. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The primary staff person or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Evacuation routes posted (3280.94(a)(5))

Evacuation routes were not posted upstairs. Requirement: The primary staff person or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that evacuation routes are posted in a conspicuous location on each floor of the facility. An emergency drill was not conducted annually in 2024. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: I wasn't aware I was to do an Emergency Drill. I was sent the form the daycare is to use. I will do an emergency drill in January. The file of child #2 lacked an emergency contact form with the addresses of the individual designated by the parent to whom the child may be released. Observed rest mats in use in the living room not feet apart. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Moved the cribs further apart and at naptime I now put one mat at one end and one in the middle when they are in use. Inspection: Renewal.
