    At time of renewal inspection on 12/10/24, Staff person #1 had not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated 1/8/19. Staff person # 1 will have until 12/25/24____ (actual date- 15 days from POC requested date) to complete the mandated reporter training. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility. Correction: Staff person #1 will be supervised at all times when working with children until mandated reporter training is competed. Staff person #1 will complete mandated reporter training and training certificate will be sent to cert rep for verification Inspection: Renewal.
