LACKING REQUIRED HIRING DOCUMENTS: Facility Person #5 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #5 may not work in a child care position at the facility. Correction: Staff will be reminded all clearances are due every five years and are to be completed before the due date. The agreement on file for Child #1, Child #2 and Child #3 were observed not to specify the services which are to be considered as extra. Requirement: An agreement shall specify the services which are to be considered as extra. The most up to date health assessment on file for staff person # 3 was observed to be greater than 24 months. This is evidenced by the health assessment being dated 9/7/2023 --10/3/2025. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Correction: Staff again will be reminded that it is a policy for staff health assessments to be completed before the 2-year due date. Inspection: Renewal. On 5/2/25 it was observed in the infant room 10 Infants with 2 staff. In room #1 it was observed that 6 YT and 1 OT was with one staff. In room #2 it was observed 2 YT and 5 OT with one staff and 8 OT with another staff. In the preschool room it was observed that 22 preschool children were with 2 staff. Requirement: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with 3270.51 (relating to similar age level). Building Surfaces (3270.76)

Based on interviews and site visit it was concluded that a sewage leak happened in the facility. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All sewage was cleaned by a cleaning company and a plumbing company replaced all of the old sewage pipes. Inspection: Complaints- Legal Location. The facility's emergency plan did not have documentation addressing continuity of operations. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Correction: For the last 3 to 5 years, the First Lutheran Church school board has been invested in addressing the continuity of operations. Inspection: Renewal. Age and Training/Health and Safety Training - Pediatric First Aid and CPR (3270.31(e)(4)(i)/3270.31(f)(10))

The file for staff person #1 DOH: 8/6/24 was observed not to have documentation of pediatric first-aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Correction: The Altoona Fire Department contacted TLC. They are planning to offer a pediatric first aid, CPR class. They will inform TLC ASAP. TLC will make every effort and attempt to monitor and remind staff of their DHS commitment. Arts and crafts non-toxic (3270.66(e))

In the preschool room it was observed in the craft cupboard shaving cream labeled "Keep out of the reach of children". Requirement: Arts and crafts materials shall be nontoxic. Correction: The shaving cream was observed by the staff and the inspector on inspection day. The inspector made mention shaving cream cannot be permitted as a craft. The agreement on file for Child #3 and Child #5 were observed to not include extra services provided. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: Presently files are being reviewed, and parents have been requested to review the agreements. Extra services will be placed on their agreements were applicable. Inspection: Renewal. The Agreement on file for Child #1, Child #2 and Child #4 were observed to not to have documentation of the dates of the child's admission to the facility. Requirement: An agreement shall specify the date of the child's admission. A child's record shall contain the dates of application, admission and withdrawal of the child. Correction: Upon being told and shown on the form where the appropriate information should be noted-TLC will complete 1st day documentation correctly. Inspection: Renewal. The facility Emergency drill log was not conducted annually. This is evidenced by the last drill being conducted on 10/25/2023. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: TLC will conduct emergency drills annually. TLC was tardy in conducting the drills within the time frame required/designated. TLC will endeavor to be more faithful. Inspection: Renewal.
