    On 4/30/26, at approximately 4:55pm, Child #1 was on the floor in the doorway between the PS and PK2 classrooms exhibiting a tantrum. Staff #1 acknowledged holding Child #1 ankles and dragging Child #1 into the PK2 classroom. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: After extensive review of job performance along with coaching and HR review, employee has been terminated from company.     On 3/14/25, at approximately 3:00pm, Staff #1 and Staff #2 noticed a red area on Child #1 forehead. Child #1 had just woken up from rest time where it appeared the plastic edge of the rest cot likely left an impression on Child #1 forehead. Staff #1 did not complete and provide an injury report to the parent on the day of the incident. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: Staff #1 wrote an incident report for the mark, which was reviewed and signed by the father of the child (see attached).
