On 4/30/26, at approximately 4:55pm, Child #1 was on the floor in the doorway between the PS and PK2 classrooms exhibiting a tantrum. Staff #1 acknowledged holding Child #1 ankles and dragging Child #1 into the PK2 classroom. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: After extensive review of job performance along with coaching and HR review, employee has been terminated from company. Staff will work alongside co-teachers whenever possible, and call admin when needed aid with challenging behaviors. Inspection: Complaints- Legal Location. On 12/23/25, at time of inspection, a bowl of marbles with a diameter of less than an inch was observed accessible to toddlers on the sink of the classroom bathroom. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. On 12/23/25, at time of inspection, Staff #1 has not completed Part 2 of PA Required Health & Safety Training for Center and Group within 90 days of hire. On 10/20/25, at time of inspection, Clorox Wipes & Sanitizing Wipes were observed accessible to children on shelf located in the PK room. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Clorox wipes and sanitizing wipes were removed from the shelves and placed in an area inaccessible to children. Inspection: Allocated Unannounced Monitoring. On 5/2/25, Director self-reported that on 4/23/25, while taking a walk around campus, a witness observed Staff #1 grabbing Child #1 arm and using harsh tone of voice while reprimanding them for hitting another child. Requirement: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: Staff #1 was given a first and final warning, along with training and guidance on appropriate redirection techniques. She will continue to work alongside tenured staff. On 3/14/25, at approximately 3:00pm, Staff #1 and Staff #2 noticed a red area on Child #1 forehead. Child #1 had just woken up from rest time where it appeared the plastic edge of the rest cot likely left an impression on Child #1 forehead. Staff #1 did not complete and provide an injury report to the parent on the day of the incident. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: Staff #1 wrote an incident report for the mark, which was reviewed and signed by the father of the child (see attached). Staff #1 was observed being rough with children by tugging/pulling on arm to redirect children and was observed picking up children and sitting them down on the floor with force. Staff #1 was observed yelling and using harsh tones when communicating with the children. Requirement: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Correction: On 2/13/25, Staff #1 was coached about using kind, calm tones with children, even when frustrated. Staff #1 was re-trained and will not use physical force in any way when redirecting a child. When faced with challenging behaviors, Staff #1 will use positive redirection and/or switch out with a co-worker if necessary. Inspection: Complaints- Legal Location. On 1/10/25, six bottles located in the infant room refrigerator were not labeled with the child's name. Requirement: Disposable nursers and bottles shall be labeled with the child's name. On 1/10/25, a plant known to be poisonous (Wondering Jew) was observed in the PK1 classroom. Additionally, various plants known to be poisonous were observed accessible to children on the outdoor play space. Requirement: Toxic plants are not permitted in a child care space. Teachers will label all plants in room. Outdoor plants identified as poisonous/toxic will be removed by landscaping. Correction: Plant in classroom was removed immediately. Inspection: Renewal. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children, by an AGS, who has completed the required training related to this citation.
