During the renewal inspection on 12/5/25 staff persons #2 and #3 did not have verification of current training in the emergency plan. Requirement: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. During the renewal inspection on 12/5/25 child #1 was observed sleeping in a bouncy seat. Age-appropriate rest equipment was not present at the facility for child #1. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. During the renewal inspection on 12/5/25 the medication at the facility for Child # 2 was not in the original box and did not contain the prescription label. Requirement: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. Correction: Parent of child #2 has taken medication to Child #2 pharmacy for proper labeling. On 12/5/25 Staff person #1 confirmed that toddlers and a preschool child (#1) were placed in highchairs at the facility during circle time in an attempt to keep them focused on circle time activities and prevent them from running around room during circle time. This occurred on 12/2/2025 Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.
