It was observed that three children were behind tables and shelving with blankets over their heads during nap time leaving them not able to see, hear or assess. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: The school aged children were moved out of these areas (behind tables and shelving) and told to take the blankets off of their heads. Inspection: Unannounced Monitoring. It was observed that two infants were sleeping in the same crib. It was observed that the children's files were in a file cupboard that did not have a locking system. Requirement: Child records are confidential and shall be stored in a locked cabinet. The refrigerator thermometer in the kitchen was not working on the day of inspection. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The refrigerator thermometer was replaced. It was observed that large foam blocks in the toddler room had bite marks and pieces missing posing a choking hazard. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. In the preschool room, it was observed shaving cream, aerosol mod podge and Alka seltzer tablets that were labeled "keep out of reach of children", in the arts and crafts cupboard.
