An unannounced inspection was conducted at the facility on April 17,2026 at that time the facility's fire drill and detection log was reviewed and the last documented fire drill was conducted on February 5, 2026. Requirement: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days. A renewal inspection was conducted at the facility on 12/16/25, at that time the facility's fire drill and detection log was reviewed and there were more than 30 days between system tests March 28, 2025 to April 28, 2025. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Child care facilities in a commercial space (center and group) must manually test its alarm at least once every 30 days and shall maintain a written record of testing with the facility's fire drill logs in accordance with 62 P.S. § 1016(c)(1). Small Toys and Objects (3270.103)

A renewal inspection was conducted at the facility on 12/16/25, at that time there were sensory bottles in the young toddler room that contained small objects that have a diameter of less than 1 inch and were accessible to children. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Comply with CPSL (3270.32(a))

A renewal inspection was conducted at the facility on 12/16/25, at that time a review of staff files was conducted. Staff person #1 did not have a NSOR on file prior to employment as is required as evidenced by NSOR dated 7/2/25. (See code sheet for date of hire). Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). TIERED LIS 1. The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). The correction date for this portion of the plan must be immediate. 2. The director will create a form to track each staff and their required clearances, trainings, and documents to assist with the hiring process and ongoing updates. This form will have due dates for all required trainings, clearances, health assessments etc. This form must be approved by the Northeast Regional Office prior to use. The director will ensure that this form is used during the hiring process and to track all required updates. This portion of the plan should reflect a date that allows for time to develop the form and implement it. Correction: 1. We will not allow any employee to start work without proper clearances. 2. We will utilize Procare to keep track of clearances and renewal dates. Staff will not work in a childcare position if they do not have all valid clearances. Emergency plan (3270.27(a)(6))

Emergency drills were conducted 2/23/23 and then not again until 8/15/24 per emergency drill logs. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: We will hold an emergency drill on or before 8/15/25. Inspection: Renewal. Consent for administration of minor first-aid required prior to admission (3270.182(5))

Child #1 and Child #2 did not have signed parental consent for administration of minor first-aid procedures by facility staff. Requirement: A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. 12 months prior to service and every 24 months thereafter (3270.151(a))

Staff person #1 had more than 24 months between physicals 6/2022 to 8/2024. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Disposable diapers (3270.135(a)(3))

Two trash cans for soiled diapers in the toddler area were not hands- free. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Written consent/Consent for emergency medical care required prior to admission (3270.124(b)(4)/3270.182(3))

Child #1 and Child #2 did not have signed parental consent for emergency medical care. Requirement: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Clean and good repair (3270.102(a))

There were 2 plastic bins, containing toys, that were cracked and broken. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
