    On 11/14/25, at time of inspection, a cracked plastic tricycle seat containing sharp edges was observed on the outdoor play space. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The director removed the broken trike immediately during the inspection and placed it outside of the play space inaccessible to children. Inspection: Renewal.     On 11/14/25, at time of inspection, the two toilet areas were not equipped with a lidded waste receptacle. Additionally, the waste receptacles in each toilet area are not removed from the facility at least daily. (Director stated trash is removed from the toilet area on a weekly basis.) Requirement: Trash shall be removed from the facility at least once per day. A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle.     On 11/14/24, at time of inspection, an EpiPen belonging to Child #1 was observed in a Ziplock bag. The EpiPen did not contain a prescription with the child's name, was not in the original box and did not contain written instructions. Requirement: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. Correction: Director obtained the original EpiPen box belonging to child #1 with prescription, written instructions and action plan. Inspection: Renewal.
