Every 60 days (3290.94(a)(1))

Fire drills were recorded on 7/1/25 then on 9/2/25 then on 11/3/25, both periods of greater than 60 days. Requirement: Fire drills are conducted at least once every 60 days. Correction: There was a gap of greater than 60 days between fire drills. I will put reminders at least every 60 days in my calendar book to have a fire drill. Inspection: Renewal. System in compliance/Operator or designated staff responsibility (3290.95(a)/3290.95(b))

The fire alarm test log showed gaps of greater than 30 days between 7/1/25 and 8/1/25 then again between 10/1/25 and 11/1/25. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met. Correction: There was a gap of greater than 30 days between fire alarm tests. I will put reminders in my calendar book at least every 30 days to test the fire alarms. Inspection: Renewal. Emergency plan (3290.24(b))

The most recent review of the emergency plan was dated 8/28/24. Requirement: The operator shall review the emergency plan at least annually and update the plan as needed. Each review and update of the emergency plan shall be documented in writing and kept on file at the facility. Correction: There was a gap of greater than 1 year in the update of my emergency plan. Services provided (3290.123(a)(3))

There was a gap of greater than 6 months in the completion of the Child Service Reports for child #1, #2, #3, #4, and #5. For child #1 the first CSR completed was dated 8/5/24 (see code sheet for start date). For child #4 a CSR was completed 8/5/24 (see code sheet for start date). For child #2, #3, and #5 a CSR was completed 6/5/23 then 8/5/24. Requirement: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e). Correction: I will put individual notes in my calendar to remind me that it is time to update the child service reports every 6 months. I will fill out a new child service report for each child every 6 months, review it with the parent, and have the parent sign and the form. Updated every 6 months/Emergency info/agreement updated 6 mos (3290.124(e)/3290.181(c))

There was a gap of greater than 6 months in the update of information on the emergency contact forms and fee agreements for child #2, #3, #4, and #5. For child #2 and #3 there was an emergency contact form and fee agreement dated 9/19/23 then 5/23/24. For child #4 and #5 they were dated 9/19/23 and 4/26/24. Requirement: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: I put a note in my calendar in January and June to have the parents update and sign the emergency contact and fee agreement forms every 6 months. I will review with the parent the emergency contact form and agreement forms, update any information that may have changed then have the parent sign and date every 6 months. If a parent has to update prior to the 6 months, I will still have them update every Jan and June to make it easier for me to keep all the forms updated at the same time. Inspection: Renewal.
