Observed in the young toddler room, a large butterfly block with a peeling cover and exposed foam and a damaged wooden shelf. Observed in the infant room, a crib with a loose side support screw. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Observed two training chairs in the bathroom of the downstairs preschool room which were not emptied and sanitized after being used. Requirement: A training chair is not a flushing toilet. A training chair shall be emptied and sanitized after each use. An acceptable sanitizing solution is 1/4 cup of bleach combined with 1 gallon of water. Observed a ceiling vent in the young toddler room and several duct work vents in the infant room which needed to be cleaned. Observed an unstable cabinet door in the infant eating area. Observed 2 exposed screw heads and an unstable drawer in the older toddler room. Observed damaged drywall by the trash can in the upstairs preschool room. Observed in the downstairs preschool room exposed drywall in an unlocked closet, damaged floor trim by the closet, and a damaged electrical cover plate. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Cleaned all ceiling vents. Tightened screws on cabinet door in infant room and evacuation crib. Replaced electrical cover in preschool prep classroom as well as placed a child proof lock on closet. Painted wall behind the trash can in the preschool classroom. Observed staff file #5 contained updated criminal background clearance and FBI fingerprint clearance which were dated more than 60 months after the previous valid clearance. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.
