A child was strapped in a highchair while staff person #1 changed a diaper in the next room. Requirement: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. Correction: The child was removed from the highchair after the child climbed up into the chair and staff secured the child in the chair. Inspection: Renewal. Staff assigned to specific children/Supervision on and facility premises (3280.113(a)(1)/3280.113(a)(2))

Staff person #1 was caring for two children during the inspection. Upon arrival child #1 was observed in the living room with non-family adults. Staff person #1 was preparing a meal while the other child sat at the kitchen table. Throughout the day, staff person #1 moved back and forth from the rooms without having the children present in the same space. A child was observed in the kitchen strapped in a high chair while a diaper was being changed in the toy room. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3280.51--3280.53. Correction: Staff keep children in view at all times and remain in the same room as all children under their supervision. When non-staff, supporting adults such as speech or occupational therapist are in attendance and working directly with a child, supervising staff remain in the same room as the children under their care. A highchair was observed in the doorway between the kitchen and toy room, obstructing the ability to use the doorway as an exit from the room. Toxic cleaning materials were observed to be stored adjacent to food in a kitchen cabinet.
