On 9/3/25 during the renewal inspection, the instructions on how to access the regulations and how to contact the appropriate regional office were not posted in a conspicuous location used by parents. Instead, there was a note located on the parent board to instruct the parents to get a binder with the regional office information that was located behind the Director's desk. Requirement: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents. The operator shall provide the parent of each child enrolled with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office. On 9/3/25 during the renewal inspection, Staff members #2-#6 did not have a completed Health and Safety training in there file. Requirement: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. On 9/3/25 during the renewal inspection, the first aid kit in the toddler classroom was filled with popcorn and trash and was in need to be cleaned. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Physical examination/Mantoux TB (3270.151(c)(1)/3270.151(c)(2))

On 9/3/25 during the renewal inspection, Staff Person #5 did not have an updated health assessment or tuberculin skin test result in their file. Requirement: An adult health assessment must include a physical examination. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. On 9/3/25 during the renewal inspection, the cups in the preschool and toddler classrooms did not have labels on the cups that were used by the children. Requirement: Disposable nursers and bottles shall be labeled with the child's name. Correction: Labels were placed on cups and a reminder to staff and parents was sent out about labeling their cups. On 9/3/25 during the renewal inspection, Staff person #1- Staff Person #3 did not have a current pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire in there file. Requirement: Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Correction: Staff member number 1 took and completed their recert of current pediatric first aid and CPR. Staff member number 3s recert was placed in her file. Inspection: Renewal. Supervised at all times (3270.113(a))

On 1/9/25 during the complaint investigation, it was observed during naptime in the Older Toddler classroom that the children slept with their heads covered with their blankets. This practice keeps the children from being fully supervised during naptime. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: On 1/13/25, All staff has taken a training on safe sleep practices for children. Each classroom will be checked by the director to make sure that there are no blankets on any of the students heads. Inspection: Complaints- Legal Location. On 9/11/24 during the renewal inspection, there was a cleaning product that was in an unlocked cabinet in the classroom located on the first floor of the facility. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.
