Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)(10))

Staff person #1 has completed first aid and CPR training, however the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum and the trainer has not provided documentation that they are PQAS-certified in that training curriculum. Requirement: Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility. Correction: Staff was hired with previous first aid and CPR certification, but instructor was not listed as PQAS certified and the curriculum was not approved. Director is PQAS certified for first aid and has scheduled a pediatric first aid and CPR training to replace the one on file that was not approved. A facility person's record was being stored on a shelf on a desk in the office. (CORRECTED ON SITE) Requirement: Facility persons' records are confidential and shall be stored in a locked cabinet. Correction: Facility person's file was on the desk while being completed. It was placed in the locked filing cabinet. Inspection: Unannounced Monitoring. Comply with CPSL (3270.32(a))

On 9/18/25, it was observed that CPSL (Child Protective Services Law) was not followed. Staff person 1 had a NSOR (National Sex Offender Registry) certificate on file that expired on 5/22/25 that was not updated until 6/12/25. Staff person 2 had a state police clearance on file that expired on 5/11/25 that was not updated until 5/13/25. Staff person 3 had a NSOR certificate on file that expired on 6/3/25 that was not updted until 6/12/25. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). On 9/18/25, it was observed that staff person 4 did not have verification of education on file. Requirement: A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: Staff person 4 was told she needed to provide proof of her education or could not continue employment at the facility. Staff 4 provided transcripts which are placed in her file. Medication log (3270.133(7))

On 9/18/25, it was observed that there was not completed medication logs on file for child 7. Child 7 will require 3 separate medication logs for their medications of Albuteral, Epi-Pen, and Benedryl. Requirement: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered. Building Surfaces (3270.76)

On 9/18/25, it was observed that facility surfaces were not in clean and in good repair. It was observed that the vents on both sides of the door between the Preschool and the Tiny Tots classrooms had a visible layer of dust. It was observed that there were ceiling tiles with water stains in the Toddler and Tiny Tots classrooms. It was observed that the counter in the Preschool room had a layer peeling off. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The vents in between preschool and tiny tots backroom were cleaned. Ceiling tiles with water stains have been replaced. The peeling surface on the preschool counter has been fixed. Supervised at all times /Staff assigned to specific children (3270.113(a)/3270.113(a)(1))

On 7/25/25, it was confirmed that child 1 eloped from the building on 7/23/25, through the door beside the cubbies, in the preschool classroom. The child wandered around the building and was found in the parking lot, approximately 3 minutes later, by a parent who returned them to the building. Staff person 1 was the primary care teacher during the time of elopement. Staff person 1 was unaware that the child was missing. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. DIRECTED PLAN OF CORRECTION: The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. DIRECTED PLAN OF CORRECTION: Management staff will conduct daily checks, at a variety of times, of staff maintaining the children in their primary care group. Management will log each staff person and their stated primary care group. Management will notate if the primary care group was complete and correctly named. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: A blind spot mirror and door chime will be installed to ensure that staff have a view of the exit door from all angles and ae alerted any time the door is opened. All staff employed by entity will be attending a minimum two hour training regarding supervision of children. Legal entity will obtain DHS approval of the training content prior to the training being scheduled. Management staff will conduct daily room checks and counts with a log to ensure all staff know and can correctly name the children and numbers in their primary care group. Logs for proof of compliance with documentation will be sent to OCDEL on a weekly basis for a time period to total 3 months. On 5/14/25, it was observed that staff person 1 did not have a health assessment on file. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Correction: Staff person 1 was contacted and told she needed to have her health assessment completed before working in a room with the children. On 1/31/25, it was observed that staff person 1 did not have two written, nonfamily references on file. Only 1 reference was available and 1 additional reference is needed. Requirement: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.
