    An unannounced monitoring visit was conducted between 1/20/2026 and 2/12/2026. On 2/4/2026, certification representative observed a high school diploma dated 6/14/2013 in the file of Staff Person #1. The certification representative noted that the formatting and appearance of the diploma differed from other diplomas from the same high school. The diploma was sent to Dieruff High School who confirmed that Staff Person #1 did not graduate from that high school on 6/14/2013. When notified that the diploma was not valid, the operator provided a CDA certificate. A valid high school diploma or GED is required to obtain a CDA. As such, the CDA cannot be accepted as proof of completion of high school. Requirement: A facility person's record shall include verification of child care experience, education and training prior to service at the facility. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. On 2/4/2026, certification representative observed pediatric first-aid/CPR training certificates dated 12/28/2023 in the files of Staff Person #1 and Staff Person #2. The certification representative noted that the formatting and appearance of the certificates differed from other pediatric first-aid/CPR certificates from the same training agency. The QR code on the documents was altered so it was unable to be scanned for verification of validity. When the certificate ID number from both certificates, which was the same on both certificates, was entered on the training provider's site, the certificate ID number is linked to a training certificate for only Staff Person #1 dated 9/13/2022 which is expired. Staff person #1 currently has valid pediatric first-aid/CPR training certificates on file dated 1/30/2026; however, Staff Person #1 was working prior to 1/30/26 with expired pediatric first/CPR training and an invalid training certificate on file. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. The training entity was contacted on 3/6/2026 and confirmed that this certificate was issued to another person. Based on the documentation reviewed, Staff persons #2 & #3 have not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): pediatric first aid/CPR training from a PQAS-approved instructor. Requirement: Staff person(s) shall complete professional development in the topics of 3270.31(f)(1 -- 10) within 90 days of hire. TIERED LIS 1.
