Exemption documentation from parent/guardian (3270.131(e)(1))

The file for Child #1 contained an immunization record that was lacking 2 doses of Hepatitis A. The file for Child #2 contained an immunization record that was lacking one dose of DTAP, one dose of HIB, one dose of Pneumococcal, and one dose of Hepatitis A. Requirement: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Correction: Child #1 For the Correction Plan, I am submitting a hand written note from the parent stating they are refusing to vaccinate. Health and Safety Training (3270.31(f))

The file for Staff #1 contained Health and Safety Part 2 training obtained more than 90 days after their start date. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Director or designated staff person ensure compliance (3270.95(b))

Upon review of documentation of testing of the smoke detection system, it was noted that the system was tested on 2.6.24 and then on 3.14.24 and 4.15.24 and then on 5.24.24 both of these increments are more than 30 days. Requirement: The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Since we cannot go back, we have changed our process on how we will schedule the testing of the smoke detector system. Inspection: Renewal. Toddler/preschool: updated health report every 12 months/Current (3270.131(b)(2)/3270.181(b))

The file for Child #1, who is a preschool aged child, contained a health assessment that was obtained more than 12 months ago. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Information in a child's record shall be kept current by the operator. Correction: The health assessment was scheduled for the child and the parent had to reschedule. We were still in compliance but going forward we will instill a better plan with our health office and family service workers. Inspection: Renewal. Comply with CPSL (3270.32(a))

The file for Staff #1 and #2 both contained Disclosure Statements that were signed after their start dates. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Since we cannot go back, we have changed our interview process and how we will have staff members sign the disclosure statements going forward. Inspection: Renewal.
