During the complaint investigation conducted on 03.26.25, it was verified parents of CH1 did not receive an incident report regarding CH1, on the same day of the incident occurrence. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: Provider will review with all facility staff via in-person and email that all incident reports must be written and given to parents the same day of the incident.
