Clean, age appropriate (3270.106(a))

At the time of the inspection, the rest equipment for children in room 205 was not labeled for the use of a specific child. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: While DHS was present, a new cot list was created and hung up by the cots. Fire safety - 1 yr. (3270.31(e)(4)(ii))

The file of staff #1 had documentation of completing fire safety training on 6-26-25 which was more than 1 year from their start date (see LIS code sheet). Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. High Chairs (3270.105)

Observed highchairs with manufactured slats for the use of should straps, but straps were not present. One highchair was grey and red and another highchair had grey elephants. Requirement: High chairs shall have a wide base and a T-shaped safety strap. Correction: The highchairs were removed and disposed of. Chairs with all straps in place were placed in the classroom. Locked or inaccessible (3270.66(a))

Observed Neutrogena sunscreen labeled "keep out of the reach of children" in a bookbag in a cubby labeled for child #3 accessible to children. Observed Aquafina and hand sanitizer in cubbies outside room F208 accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The items were removed from the bags immediately. Parents were also emailed again and reminded that any item that is labeled as toxic to children cannot be left in the bags at daycare. These items either need to remain at home or be given to the staff so they can put them in a lockbox with the proper documentation. Inspection: Renewal. Emergency Numbers Posted (3270.74)

At the time of the inspection, the phone number for the ambulance was missing in all child care spaces. Requirement: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility. Correction: New Emergency Contact Phone numbers were made and posted to include the non emergency number for ambulance. These were then hung in all classrooms and placed in all emergency bags. No peeling paint or plaster (3270.77(a))

Observed peeling paint an rust on the square, red picnic table in the outdoor play space. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Handwashing signs (3270.82(h))

At the time of the inspection, there were no handwashing signs posted at the sinks and toilets in the boys' restroom. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: Signs were hung up in the bathroom following the noncompliance. The director emailed the GJCTC building administrator and talked with a maintenance worker so they are aware that these signs need to remain up. Inspection: Renewal. Stairs, exits, etc. unobstructed (3270.91(a))

At the time of the inspection, observed in rooms 205 and 206 interior doors blocked with wooden shelving units. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: All shelves that were blocking the doorways have been moved to allow access to the doorways in case of an emergency. Inspection: Renewal. Clean and good repair (3270.102(a))

Observed in room 204 a white wooden table in the kitchen play area with loose legs. Observed in the indoor gross motor space, a plastic blue truck with broken plastic around the tire. Observed in the preschool room, a small round table with 3 loose legs. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The blue truck was immediately thrown away. The wooden tables legs were tightened and no longer has wobbly legs. Inspection: Renewal. Infant sleep position (3270.119)

Observed child #1 sleeping in a bouncer rather than in a supine position according to the recommended by the American Academy of Pediatrics. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. Services proceeded (3270.123(a)(3))

The files of children #2, #3, #4, and #5 lacked child service reports every 6 months. Requirement: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information). Correction: To ensure that all children get an updated child service report every six months we will maintain the excel spreadsheet. Inspection: Renewal. Each child care space (3270.124(c))

At the time of the inspection, observed child #3 in the gross motor space but a copy of the child's emergency contact form was not present. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: Child #3's emergency contact information was immediately printed out and put in the binder of the young toddler room. Inspection: Renewal. Toddler/preschool: updated health report every 12 months (3270.131(b)(2))

The file of child #2 had health assessments more than 12 months apart due to the fact they were dated 9-8-23 and 11-13-24. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. Correction: To ensure that all children from birth - 2 get an updated health assessment every 6 months and two years and up once a year we will keep an excel spreadsheet. Inspection: Renewal. Written statement (3270.166(1))

At the time of the inspection, there was not an infant feeding schedule for children #6 and #7. Requirement: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent. Correction: The provider immediately printed off the infant feeding schedule and gave it to the parents to fill out.
