Correction: On 10/30/2025, a new plastic-lined, hands-free covered diaper disposal can was purchased and placed in the Older Toddler area. Promote development/Infant/Toddler Stimulation (3270.111(c)/3270.112)

During a renewal inspection conducted on 10/28/25, two of the infants were observed multiple times awake in their cribs. Only one infant, who was not feeling well, was observed being held, rocked, talked to, played with or carried by staff. The layout of the room is not conducive to the infants playing, crawling or interacting with each other. Requirement: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background. Infants and toddlers shall be provided stimulation by being held, rocked, talked to, played with and carried. Correction: On 10/29/2025, the infant classroom layout was rearranged to provide more open floor space for movement, play, and interaction. Age-appropriate toys and soft mats were added to encourage crawling, rolling, and exploration. Staff were retrained on the importance of active engagement with infants including: holding, rocking, talking, and providing stimulation throughout the day to promote developmental growth and bonding. During a renewal inspection conducted on 10-28-25, there was not 2 feet on three sides of the cribs that were in use. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: On 10/28/2025, the cribs were immediately rearranged to ensure that a minimum of 2 feet of space is maintained on three sides of each crib while in use. The room layout was adjusted to allow for safe access, emergency evacuation, and proper air circulation around all cribs. The director verified the spacing with a measuring tape to confirm compliance with regulations. In addition, rooms in the facility were arreange. During a renewal inspection conducted on 10-28-25, chipped paint was observed on the wall across from the entrance to the indoor play space. There is also peeling paint on the baseboard in the boys' restroom. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: On 10/31/2025, after all children were dismissed, the affected areas with chipped and peeling paint were scraped, sanded, and repainted using a non-toxic, child-safe paint. The facility ensured that all paint repairs were completely dry before children re-entered the areas. During a complaint investigation conducted on 9/25/24, all staff files were reviewed. It was determined that it has been more than 60 months since Staff Person #3 and Staff person #5 updated their state police clearances. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. Facility Persons #3 & #5 may not work in a childcare position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. The CPSL was revised on 9/18/2018 to include a requirement that all clearances be updated and on file at least every 60 months. The Child Protective Services Law (CPSL) was revised to include the requirement for the National Sex Offender Registry (NSOR) Clearance effective 9/30/2019. The CPSL also states that anyone hired after 9/30/19 was required to have the NSOR clearance to work in childcare. Current staff must have the NSOR clearance no later 7/1/20. CPSL was revised to include new hires that have resided out of Pennsylvania with in the 5 previous years must obtain State Police and Child abuse clearances from the state that they have resided. During a complaint investigation that was conducted on 9/25/24, all child files were reviewed. A handwritten note with no date was found describing an incident in which a school age child pulled another school age child's pants down & hurt this child. No corresponding incident report was found for either child. There is no documentation that either child's parent was notified. Requirement: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. Correction: When the incident happened between both school aged children, the center director spoke directly with both children their parents about what had occurred. School age child #1 described to me that he wanted to get the ¿thing¿ the other school age child had which was a figget. During my conversation with the child that pulled the other school age student pants down, he said his intention was not to hurt the other child or put the pants down, he just wanted to reach the toy.
