Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: Person whom had previously provided Staff person #1's experience was contacted to verify total number of hours. The total hours of experience from said person was 1,920 hours. Time cards were pulled to combine hours and now staff person #1 has a total of 2,983 hours of experience, making her an Assistant group supervisor. Inspection: Renewal. Protective Outlet Covers 5 yrs. or less (3270.65)

In the Pre-K 2 room, an outlet near the lunch/snack table was observed to be missing a receptacle cover. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Correction: After noticing the protective receptacle cover was not plugged into the wall, a new cover was put into the outlet immediately. Inspection: Renewal. At several sinks in the facility, the hot water temperature exceeded 110 degrees: Infants 117.3, Young Toddlers 114 and Pre-K 2 112.8. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: Upon realizing that the hot water temperature was above 110 degrees, the administration team turned the hot water tank down. Inspection: Renewal. Building Surfaces (3270.76)

In the outdoor play area on the right, four exposed bolts were observed at the corner of the fencing nearest the sandbox. Another two exposed bolts were observed at the corner nearest the retaining wall on the same side of the play area. Both areas create a potential hazard to children at play. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Small Toys and Objects (3270.103)

In the Older Toddler room on a shelf of art supplies by the window, a red bin was observed to contain crayons. Numerous broken crayons were observed to be in pieces smaller than one inch, creating a potential choking hazard. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. In the Pre-K 1 room near the dress up supplies, a small table was observed to have loose legs, causing it to wobble and be unstable. In the Pre-K 2 room, a small gray table was observed to have loose legs, causing similar instability. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: Once the wobbly table(s) were noticed, a staff member tightened all legs on the tables until it was sturdy and fixed. Inspection: Renewal. Admission date (3270.123(a)(6))

The Agreement on file for Child #1 did not include their date of admission. Requirement: An agreement shall specify the date of the child's admission. The health assessments for Staff Person #3 appeared altered when presented for inspection. Specifically, the health assessment dated 5/5/2023 appeared to have had the date altered. Upon further investigation, it was confirmed with the health provider that all health assessments documented after 2008 for this individual had been forged. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Requirement: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The statement shall be kept in the child's record.
