The cold air register in the upstairs toddler 2 room on the floor by the diaper table was bent up causing a tripping hazard. In the preschool room under the TV the rug was fraying. In the outside space the dryer vent pipe coming out of the wall was cracked causing a pinching hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
