It was observed in the preschool three's classroom, diapers were not being discarded into a plastic-lined, hands-free covered trash can. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: A hands-free, covered trash can with a plastic liner was immediately placed in the diapering area. Staff were instructed to begin disposing of all diapers in the appropriate container without delay. Inspection: Allocated Unannounced Monitoring. Supervised at all times (3270.113(a))

It was observed in the preschool four/five's classroom that children laying on their nap cots were not able to be properly supervised by staff. The nap cots were placed in areas that caused children to be out of sight from staff. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Correction: Classroom layout was rearranged so that all children can be supervised at all times and staff are now able to see every child while they are on their nap cots. It was observed in the preschool four/five's classroom that nap cots were not at least 2 feet apart on three sides while in use. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Nap cots were immediately rearranged to ensure a minimum of two feet of space on three sides of each cot. Classroom layout was adjusted to maximize available space while maintaining proper supervision and accessibility. Staff verified spacing prior to the next scheduled rest period. Handwashing signs (3270.82(h))

It was observed in the bathrooms inside the preschool three's and preschool four/five's classrooms did not have handwashing signs posted at each toilet and sink. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. Correction: Age-appropriate handwashing signs have been printed and posted at each toilet in the affected classrooms/bathrooms. Locked or inaccessible (3270.66(a))

It was observed in the preschool three's classroom, Swiffer Wet Jet solution was in the Swiffer mop stored in the corner of the bathroom and accessible to children. It was also observed in the preschool three's classroom, Lysol, Clorox cleaner, Clorox wipes, and Swiffer Wet Jet solution were being stored under the sink that was not locked and accessible to children. It was also observed in the preschool three's classroom, Shaving Cream, a bar of soap, and Multipurpose Solution were being stored in an unlocked drawer and accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The Swiffer mop was removed from the classroom. The under-sink cabinet and drawer were secured with childproof locks the same day of observation. All cleaning products, chemicals, and non-child-safe materials were immediately removed from accessible areas. Items were relocated to locked cabinets and/or placed in secured storage areas out of children's reach. Inspection: Allocated Unannounced Monitoring. The file of staff person #1 contains a NSOR verification dated 10.16.25. During an inspection of this record, Staff Person # 2 confirmed that staff person #1 was working with children on 8.4.25 without the required verification on file. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Persons may not work in a childcare position at the facility without the required clearances and verifications on file. Correction: The original NSOR request was sent in before the staff person was scheduled to work. It was realized on 10/1/2025, that Staff Person #1 was still missing their NSOR clearance. Another copy of the NSOR clearance was immediately requested. A copy of the completed NSOR result has been printed and placed in Staff Person #1's personnel file. Inspection: Renewal. Locked or inaccessible (3270.66(a))

During the completion of the renewal inspection, it was observed that a handcart in the main hallway [outside the kitchen area] had disinfecting wipes and other cleaning products labeled 'Keep out of Reach of Children' within accessible range of children potentially using the hallway area. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The center was undergoing drywall repairs in the kitchen and several classrooms. The contractors intended to complete the work in one day, coinciding with our staff training, but it ultimately took them several days to finish. During this time, a kitchen cart filled with supplies was moved into the hallway by the contractors. These items are typically stored in the kitchen and kept out of children's reach. When our certification representative saw the cart, it was promptly returned to the kitchen to ensure it remained inaccessible to children. A memo was submitted to staff reminding them that all cleaning/hazardous material supplies must be kept out of reach of children at all times. Inspection: Renewal. Upon completion of the physical site inspection, it was found that a written plan of daily activities was not posted in the school-aged childcare space. Requirement: The written plan of daily activities shall be posted in the group space. Correction: Each classroom keeps an updated daily activity plan. The school age classroom lost its dedicated supervisor, and as staff rearranged the room, wall décor---including the activity plan---was removed during October training and not replaced. The file of Staff person #1 does not contain a completed DHS required FBI clearance [DOE clearance dated 9.11.24 was found on file] or a valid NSOR verification. An application for the NSOR verification was found on filed dated 5.5.25, but not application was found for the DHS FBI clearance. Staff Person #1 was hired provisionally, which ended on 6.20.25. The facility owner confirmed that Staff person #1 was currently working on the date of investigation. Requirement: Facility person #1 must be removed from a childcare position by the close of business [Effective 7.1.25]. Correction: Provider removed facility person #1 from the facility until her FBI Fingerprint and her NSOR were obtained on July 16, 2025. Facility person #1 was able to return to the classroom on July 21, 2025. Assistant Director went on maternity leave shortly after facility person #1 was hired. Age and Training (3270.31(e)(4)(i))

Staff person #2 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring (6.19.25). Documentation of updated Pediatric first aid and CPR training is not on file and the facility owner confirmed that Staff Person #2 was currently working with children. Requirement: Competence is the completion of training by a professional in the field of first aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required training has been completed, staff person #2 must be supervised, when interacting with children at a minimum by, an (AGS) who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #2, staff person #2 may not work in a child-care position at the facility. Correction: Staff person #2 was on vacation until July 28, 2025. Staff person #2 completed Pediatric First Aid and CPR training on July 28, 2025. Provider has scheduled all staff persons that CPR training is due to expire in 2026 for February of 2026, to ensure that staff person's training will not expire prior to completing the course. Upon inspection of personnel files, it was found that Staff Person #1 had a health assessment dated 3.23.21 [expired 3.23.23] AND it was found that Staff Person #2 had a health assessment dated 8.12.22 [expired 8.12.24]. Requirement: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis. Correction: Health Assessment forms were provided to Staff Member #1 & Staff Member #2. Appointments have been made for their health assessment to be completed by their physician as soon as possible. ACIP recommended immunization record /Dismissal policy (3270.131(e)/3270.131(e)(3))

Upon review of children's files, it was found that the immunization record for Child #1 included only one dose of Pneumococcal vaccination [missing 2-3 doses] AND had no record of any MMR, Varicella or Hepatitis A vaccinations, per the ACIP recommendations. Requirement: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Correction: Child #1 has provided the appropriate vaccination record to show that they are in compliance with this regulation. A copy was provided after the inspection. The family is on a delayed vaccination schedule but is waiting for an appointment to get the exemption letter from the physician. Inspection: Renewal. Locked or inaccessible (3270.66(a))

Upon inspection of the physical site, it was found 1. In the School-aged childcare space, one bottle of hand lotion and one bottle of saline solution [labeled 'Keep out of reach of children'] were accessible to children in care [no children were in the space at the time of inspection]; 2. 3 bags of potting soil [labeled 'Keep out of reach of children'] were on the ground and accessible to children playing in the outdoor space. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: The school age classroom teacher has signed a plan of action to make sure that chemicals, lotions and other such items are kept out of reach of children. She will check the classroom each morning when she arrives and each evening when she leaves to make sure all items that are hazardous for children are put in a locked cabinet or out of reach of children. The bags of potting soil have been put into the locked area on the playground. The director will do regular playground inspections to make sure that things on the playground are safe and in good working order and all things that should be out of reach of children are stored in the appropriate location.
