A water-stained ceiling tile was observed in the hallway and room #5. The bolts on the toilet in the bathroom off room #2 were not covered. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. An open storage container with chickpeas, which are less than an inch in diameter, was observed in the older toddler area of the main room, and according to staff, the chickpeas were used by the older toddlers for an activity designed to develop their fine motor skills. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.
