During renewal inspection, staff files were reviewed. Staff #1's previous mandated reporter training is dated for 3/11/20. An updated training was not completed until 4/30/25. This date indicates that the mandated reporter training was not completed within the 60-month requirement. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Staff 1 has completed the mandated reporter training, and it will remain in the file. Inspection: Renewal. Source: PA DHS COMPASS provider search (compass.dhs.pa.gov).

Devices must be compliant/Director or designated staff person ensure compliance (3270.95(a)/3270.95(b))
During complaint investigation, it was observed that the following dates indicate that fire alarm testing was not done within the 30-day requirement: 11/1/24, 12/2/24, 1/2/25, 2/3/25, 3/3/25, 4/3/25. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: We have a very detailed system for maintaining monthly fire drills and within the 30 day guidelines. No system is perfect, and neither are our directors. The fire drills in question were one day late and one was miscounted due to a holiday. Our plan of correction will not include changes to a system with no issues. We will be more meticulous in counting the 30 days and will have a 2nd person to ensure the fire alarm testing is handled within the 30-day requirement. Inspection: Unannounced Monitoring.
