Small Toys and Objects (3270.103)

As a result of the deuterating ground surfacing in the facility play space there are hundreds if not thousands of rubber pellets on the ground of the facility play space which can be a hazard for children still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The facility will blow the playground every Monday morning before children goes outside. In addition, the facility purchased poured in place rubber turf to fill in any hole that exists and have purchased rubber turf sealant to apply over the whole playground surface. Building Surfaces/No peeling paint or plaster (3270.76/3270.77(a))

At time of inspection, there was chipped paint on the walls in rooms # 1, 3, 6 and 7. In addition, the cubbies outside of room #9 were not mounted to prevent possible falling hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: The paint in all rooms mentions shall be repaired and there shall be no chipped paint and/or plaster in facility. At time of inspection, the parents of child # 2 declined immunizations for their child and did not have letter of exemption documented in their record. Requirement: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record. Correction: The parent of child #2 shall provide a letter of immunization exemption for their child documented in their child's record as required. Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports (3270.131(b)(2)/3270.182(1))

At time of inspection, children #3 (last health assessment-8/14/24) and #4 (last health assessment-10/22/24) did not have updated health assessments documented in their records. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: The parents of both children shall provide and updated health assessment to be documented in their records as required. Inspection: Renewal. At time of inspection, the parents of child #1 did not provide the health insurance policy number and address of release person documented on their child's emergency contact form. In addition, the parents of children #1 and #2 did not have phone number of release person documented on their children's emergency contact form. Requirement: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: The parents of both children shall be required to provide the missing information including the policy number, address and phone number of release person and document it on their child's emergency form. Refrigerator (3270.107)

At time of inspection, the refrigerator in room #1 had a temperature reading of 50 degrees. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The refrigerator in room #1 was lower when it was brought to our attention that it was reading too high and was re-inspected to ensure the temperature meets required reading of 45 degrees or less by conclusion of inspection visit. Clean, good repair, proper size (3270.104(a))

At time of inspection the child size blue couch in room #10 was torn and in need of repair and/or replacement. In addition, the rugs in rooms # 3 and #8 were soiled and in need of a deep cleaning. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: The blue child size couch in room #10 shall be repaired and the rugs in rooms #3 and 8 shall be deep cleaned and sanitized. Inspection: Renewal. Unsafe Areas in Outdoor Space/Building Surfaces (3270.63/3270.76)

At time of inspection, hazardous areas were observed in the outdoor play area used by the children. The area was attempted to be restricted by several plastic toy chest. However, the toy chest did not prevent children from being able to climb over the chest and into the area that could be restricted and is a falling hazard to children. Requirement: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The facility shall do a better job of blocking off the area with safety netting being installed to prevent children from being exposed to a falling hazard. This area shall be screened off all the way to prevent children from accessing the area to prevent any possible hazard and/or injury. Inspection: Renewal.
