Refrigerator (3270.107)

At time of unannounced inspection on 5/5/26, a thermometer was not present in the refrigerator in the Toddler #1 room to ensure the temperature remains at 45 degrees Fahrenheit or less. Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: A new thermometer will be purchased and placed in the refrigerator in the Toddler #1 room. Health and Safety Training/Health and Safety Training - Pediatric First Aid and CPR (3270.31(f)/3270.31(f)(10))

At time of renewal inspection on 10/7/25, the first aid and CPR certification on file for staff person #2 and #3 was not conducted by a PQAS trainer and the certification for staff person #2 was not granted through an approved curriculum. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire. Correction: Staff person #2 and #3 will be notified they need to retake Pediatric First Aid and Pediatric CPR, and the training will be conducted by a PQAS certified trainer using an approved curriculum. Building Surfaces (3270.76)

At time of renewal inspection on 10/7/25, a ceiling vent in the Pre-K room was observed to be visibly dusty and in need of being cleaned. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Clean and good repair (3270.102(a))

At time of renewal inspection on 10/7/25, the nap mats in the Pre-K room were observed to have torn edges and exposed foam. The large foam blocks in the young toddler room were observed to have torn edges and exposed foam. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The foam blocks will be removed from the room immediately and not used by the children. The director ordered new nap mats and the order got cancelled. Age and Training (3270.31(e))

At time of renewal inspection on 10/7/25, staff person #1 did not have documentation of completing 12 clock hours of annual child care training. Requirement: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Correction: Staff person #1 will complete an additional 6 hours of child care training so there will be a total of 12 clock hours on file. Inspection: Renewal. No peeling paint or plaster (3270.77(a))

At time of renewal inspection on 10/8/24, chipped white paint was observed on the wall trim in the young toddler room. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
