At the time of the unannounced inspection on 11/20/25, it was found that staff #1 grabbed child #1 and tapped child on the arm. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Facility will ensure that all staff not use any form of physical punishment, including spanking a child. Facility provided policy regarding supervision to the regional office.
