An inspection on 5/20/26 revealed that staff person #1 was informed by a parent that individual #2 was hitting and pushing a child aggressively in the hallway of the facility. Staff person #1 did not report to Childline as mandated. Requirement: An operator or a staff person who has reason to believe that a child enrolled in the facility has been abused is required to report suspected child abuse to ChildLine as mandated by the CPSL. A review of video footage from 3/5/26 revealed that staff person #1 hit child #1 in the face with the back of her hand. Requirement: 1.A facility person may not use any form of physical punishment, including spanking a child. 2. Staff at the facility are required to attend the Safe Reactions training provided by the ELRC. Correction: 1. 3/12/26 Immediately staff will not use any form of physical punishment. Staff member #1 is no longer employed at SFA. Provider will ensure that all staff is trained on all safe reactions in a classroom upon hire and throughout tenure. 2. 5/11/26 Provider will ensure that all staff is trained on Safe Reactions provided by the ELRC. A review of video footage from 3/5/26 revealed that staff person #2 was sitting at a table with their back turned to the children for an extended period of time. During the time staff person #2 was sitting at a table with their back turned to the children - child #1-bit child #2 on the face. Child #1 was assigned to staff person #2. Requirement: 1.Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. 2.Staff at the facility are required to attend supervision training provided by the ELRC. Correction: 1. 3/12/26 Immediately staff will provide direct supervision at all times and know the names and whereabouts of the children in their assigned groups. 2. 5/11/26 Provider will ensure all staff is trained correctly on classroom management and supervision provided by ELRC. During the renewal inspection on 10/17/25 the gate to the older preschool and school age playground had exposed bolts. (Missing thread covers) Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. During the renewal inspection on 10/17/25 the outside classroom door on the playground had areas of missing/damaged paint. During the inspection on 7 25 25 a review of the fire alarm sounding log revealed that the fire alarm had not been sounded since 614 25. During the renewal inspection on 9/18/24 a gap between the artificial grass and unitary play surface- causing a tripping hazard - was observed on the early learning center playground . During the renewal inspection on 9/18/24 staff person #12 was observed walking 7 preschool children around the block.
